MEPCOM FEE BASIS PROVIDERS

ATTACHMENT 1

QUESTION AND ANSWER DOCUMENT

PAGE AND PARAGRAPH OFFEROR’S QUESTIONS GOVERNMENT’S RESPONSE

1. Page 38 – Last Paragraph

Will the government please clarify what Clauses and Reps and Certs offerors are responsible for filling out and submitting?

Page 115, (5) Representations and Certifications, states what shall be completed and submitted for the Representations and Certifications. For the clauses, it is the responsibility for all interested offerors to go through the solicitation and fill out each applicable clause.

2. Page 92 – Attachment

Is it the intent of the government for offerors to provide the company name in the file names of the Management and Technical Approach and the Scheduling Capabilities as specified in Attachment 2? Please clarify for the electronic files under the sanitized versions?

No, it should be Cage Code. This was corrected with this amendment.

3. Page 114 – Proposal Table of Contents

Will the government please clarify if the Price Proposal needs a table of contents and glossary of abbreviations and acronyms?

No, the Price Proposal does not need a glossary or abbreviations and acronyms. If it is not listed in the chart, then it is not needed. Additionally, under the Proposal Table of Contents chart, the solicitation goes into detail concerning each Volume that is required and exactly what is needed to be provided for each.

4. Page 114 – Proposal Table of Contents

Will the government please provide the file name to be used for the pricing narrative?

The pricing narrative is a part of the price proposal.

Please go to page 119 under Volume IV. It outlines the titles under this Volume.

5. Page 91 -Attachment 2 Proposal File Naming Convention and Page 112 -Addendum TO 52.212-1 (1) Additional Restrictions

On page 91, the Technical Quality Proposal lists separate electronic files using the document titles listed. However, on p. 112, the Government states they want the Technical Quality Proposal in one file (Technical Quality Proposal (Volume II) is limited to 50 pages and shall be one complete word document.). Which instructions does the Government prefer?

The Technical Quality Proposal needs to be in one file just as it is stated on page 112. This is to ensure that you do not go over the 50 page limit.

The separate titles are for you to separate the sections and address each area within the file. Please read Additional Restrictions, page 112, para (c) (2). The Table of Contents and Abbreviations can be separate files as they do not count towards the 50 page limit.

6. Pages 94-95 - Attachment 6 and Page 112 – A. Proposal Submission; 1.

Introduction

Does the last bullet on page 112 under section (2) “Points of Contact List completing Past Performance Questionnaires” refer to Attachment 6? Is this considered outside of the page count?

Yes, Please read Additional Restrictions, page 112, para (c) (2), which outlines the files that will not count against any page limits.

7. Page 111 – last paragraph Addendum To 52.212-1

Will the government accept hand delivery of the proposal?

Yes, as long as it is delivered before the due date and time.

8. Page 3 - Minimums and Maximums

The RFP states, “the estimated MAXIMUM value of all orders issued shall not exceed $161,000,000.00.” What factors were considered in determining the maximum value? Will submissions with pricing in excess of $161,000,000.00 be

The maximum value of the contract is based on the maximum quantities the Government intend to order under this contract.

The maximum value amount reflects the maximum value of the contract and not the maximum amount for pricing purposes.

eligible for consideration?

9. Page 117 - Subfactor 1B: Management and Technical Approach

Is it up to the contractor to determine who is Key Personnel?

Yes.

10. Page 138-

2. Performance Risk Approach

Please clarify the statement on page 138 that states: “Past performance regarding predecessor companies, prime Offeror, key personnel who have relevant experience, or major sub-contractors, will be rated as highly as past performance information for the principal Offeror.”

First, the statement that information from other than the prime offeror “will be rated as highly as past performance information for the principal offeror” would mean that if a principal offeror submitted on contract which received the highest rating and two contracts from subcontractors which shows poor performance, then all of the past performance would be given the highest rating.

The statement means exactly what it states: All past performance submissions utilizing any predecessor companies, key personnel or major sub-contractors will be rated as highly as past performance information for the principal offeror.

No, it does not mean “that if a principal offeror submitted on contract which received the highest rating and two contracts from subcontractors which shows poor performance, then all of the past performance would be given the highest rating.”

The Government will use past performance information collectively (i.e., principal offeror, major sub-contractor) to determine a performance confidence assessment rating (e.g., substantial confidence, satisfactory confidence, etc.).

11. Page 114 – Volume I Business Proposal

2. Table of Contents lists “Volume I – Business Proposal shall contain a Master Table of Contents referencing the contents in all volumes.

No, Please refer to the Proposal Table of Contents chart starting on page 113 and the narrative that begins at the bottom of page 114 as to the

The Master Table of Contents will not count against the page limitations for their respective volumes.” Is the Master TOC only required in Volume 1? Or is it required in respective volumes?

items that go with each volume.

12. Page 116- Technical Quality Proposal Instructions

Please clarify this contradiction in the RFP:

On page 116 of the technical quality proposal instructions, it states that “The technical proposal shall not contain cost/pricing, past performance or incumbency information.”

This instruction is contradicted by the information on page 117 that requires an offeror to:

“Describe in detail your company’s organization, key personnel and their qualifications and responsibilities to successfully execute this contract.” How can an offeror describe in detail its key personnel and their qualifications without mentioning past performance and incumbency information? Also, if key personnel are currently performing on the contract, how can an offeror address their availability without stating

The offeror must describe in detail their company’s organization, key personnel and their qualifications without stating past performance and incumbency information. This can be explained without stating what past contracts the key personnel worked on, which would include their title, qualifications and responsibilities to successfully execute this contract.

that they are currently on the job?

13. Page 115 -Volume II – Technical Quality Proposal

The RFP lists “Summary Section” excluded from page count in Volume II - Technical. Will the government please confirm that this is a traditional brief Executive Summary?

It is the offeror’s choice as to how or if they want to write a Summary Section.

14.Page 136 – B. Evaluation Approach

The RFP evaluation language states, “Offerors must ensure all information pertaining to the factor or sub-factor is addressed in that particular portion of the proposal. Offerors are cautioned that only the information included for each factor/sub-factor will be evaluated. For example, only information included in the section labeled “Management and Technical Approach” will be evaluated for Management and Technical Approach. If management and technical approach information is included elsewhere in the proposal, it will not be evaluated as part of the Management and Technical Approach sub-factor.”

Can the offeror assume that the summary information provided as the “Summary Section” (excluded from page

The Government will evaluate the information included for each factor/sub-factor within the 50 pages of the Technical Quality Proposal.

count) for each subfactor of the technical will be evaluated and included within each subfactor for the digital file submissions (Attachment 2, page 91) ?

In the Proposal Table of Contents, Volume II, Technical Quality Proposal, the “copies” column provides the following instruction:

NOTE: 3 CD-Rs must be sanitized, to include the label/cover of the CD-R which should reference the Offeror’s CAGE code instead of the name of the company (See Solicitation Attachment 2) Note: Proposals that include contractor specific information, i.e.

company logos, will not be evaluated.

Question: Does this mean that in three of the CD-Rs for Volume II, any

Yes reference to the offeror’s name must be redacted or replaced with a neutral term such as “Offeror”?

Subcontractor/Teaming Partner Consent Letters

The instruction refers to Attachment 4 “Subcontractor/Teaming Partner Consent Letter.”

Question: Please confirm reference should be to Attachment 3, which is the Sample Subcontractor/Teaming Partner Consent Letter.

Yes, the reference should be Attachment 3. It was changed with this amendment.

18. Page 118, 4.

Performance Assessment Information

The instruction states:

The offeror shall list all performance assessment submissions in the format referenced on the attached excel spreadsheet, Attachment 5, entitled, “Performance Assessment Information”.

Question: There was no Attachment 5 provided as Excel spreadsheet. Will that be provided?

There is a sample Attachment 5 (See page

95) listed within the solicitation. Use that example and input it on an excel document.

19. Page 115 – Section 6 The RFP states “the Offeror shall explain how they plan to have or obtain adequate financial resources to support any awarded contract and resultant TOs”. What is the minimum Line of Credit the Prime Offeror must maintain for the life of the contract?

There is no minimum line of credit. For evaluation purposes, the contracting officer will determine if an offeror’s financial resources are adequate enough to support any awarded contract.

20. Page 117 – Volume III Performance Risk Proposal Instructions

It states no more than three PASs may be submitted. It also states

The Government will evaluate past performance information for major sub-that all three of the PASs may come from a major subcontractor. Is the government suggesting that the prime offeror does not have to have any past performance or experience providing services related to the services required in the solicitation? If the prime offeror is not required to have any past performance, how will the government validate that the prime offeror is providing at least 51% of the work?

contractors whether or not the prime offeror has past performance information.

The Government will use the information in the Subcontractor/Teaming Partner Table and other information in the offeror’s proposal to determine prime offeror’s and major sub-contractor’s roles and responsibilities in performing the work. By submission of an offer the offeror agrees to comply with the Limitations on Subcontracting Clause.

21. Page 118 – RFP Paragraph 2(b)

Will the contractor officer conduct and document the prime offeror’s ability to comply with the subcontracting limitation?

See answer to question 19.

22. PWS 1.4, Page 53 When contractor FBP provides services as CMO in the absence of government CMO, are these hours billable?

Yes, at the DPC 4 amount.

23. PWS 1.11, Page 54 Are the FBP Meeting Attendance hours billable to the Government?

24. PWS 4.14.1, Page 70 PWS 4.14.1 states “The contractor shall designate two physicians, one for each sector, to screen and interview all potential candidates…” Can the Government define what is meant by “each sector”?

USMEPCOM is divided into 2 sectors with the dividing line being the Mississippi River. A map has been included as an exhibit (See Exhibit A, which was provided as a separate Power Point document posted as part of the solicitation).

25. PWS 5.8, Page 72 Are there any specific hardware/IT requirements needed on the part of the

26. PWS 5.9, Page 73 PWS 5.9 says that “The COR will assign individuals (Government Points of Contact) at each MEPS to be responsible to provide the contractor the number of FBPs needed for the next business day, with at least a 12-hour advance notice prior to the required shift.” Assuming that the 12-hour advance notice is the minimum, how much notice will be given to the contractor on average?

On average, 24 to 48 hours.

27. N/A Can the Government provide the expected distribution of hours/percentage of examinations provided by physicians versus mid-level providers (i.e. Nurse Practitioners or Physician Assistants)?

No, that statistic is not tracked by the government. FBPs are expected to work at the 4 point per hour average.

Male <39 equals 1 point, female <39 equals 2 points per hour.

28. PWS 5.10.5.12., Page
