About this file

The document is a Past Performance Information Sheet (PPI) template for Solicitation W81K0025QA135 for Trauma Registry Services issued by the Department of the Army Medical Command. The template requires offerors to provide detailed past performance information, including company details, contract numbers, points of contact, labor categories, location of services, number of full-time employees (FTEs), work description, contract award amounts, and project status.

The solicitation requires submission of the completed PPI by 14 July 2025 at 1400 CST, with preliminary questions due by 8 July 2025 at 1400 CST. The PPI template is designed to capture comprehensive information about previous contract performance, allowing the government to evaluate a contractor's capability and track record in delivering similar services. Offerors are instructed to provide up to three pages of information per project, with specific guidance on referencing task order numbers for IDIQ (Indefinite Delivery/Indefinite Quantity) contracts and documenting base and option year contract values.
