About this file
This document is a Questions and Answers (Q&A) file related to a PFAS (Perfluoroalkyl and Polyfluoroalkyl Substances) blood testing requirement for firefighters. The Q&A addresses three key points: confirmation that this is not a medical surveillance study but an exposure assessment, verification of serum sample testing, and updates to the Performance Work Statement (PWS). Specifically, the document indicates that two Department of Defense Medical Treatment Facility guides have been added to section 6.0 of the PWS, and the list of compounds for testing has been expanded from 6 to 13. The testing is designed to record PFAS concentration in firefighters' occupational health records, with medical staff potentially providing serum samples after centrifugation. The document is dated 8/19/2025 and provides responses to clarifying questions about the testing protocol.

1 Is the study based on analyzing a serum sample matrix? This is not a study, so more information may be required to answer this question sufficiently. Please see the publications below. PFAS testing is not part of firefighter medical surveillance or qualification, but is done solely to assess exposure. This requirement is only to offer blood (serum) testing for select PFAS compounds and record the testing results of the concentration of PFAS in the firefighter’s OH record.
The two new publications for reference have been added to section 6.0 of the PWS are:
1. Department of Defense (DoD) Medical Treatment Facility (MTF)1 Guide for Ordering Laboratory Analyses for the Detection of Perfluoroalkyl and Polyfluoroalkyl Substances (PFAS) in Blood or Serum.
2. Department of Defense (DoD) Medical Treatment Facility (MTF)1 Guide for Ordering Laboratory Analyses for the Detection of Perfluoroalkyl and Polyfluoroalkyl Substances (PFAS) in Blood or Serum.

2 Will the samples provided by medical staff be the serum fraction after centrifugation?
This can be done, if that is the recommended solution. Ensure that any limitations of your quote are specified.

3 Has the PWS been updated. Yes. The publications have been added with links in section 6.0, and the list of compounds for which testing is required has been increased to 13 from 6.
