About this file
This is an analytical laboratory report documenting per- and polyfluoroalkyl substances (PFAS) testing for water samples collected from Toledo Air National Guard Base's overflow tank.
The report, generated by Eurofins Cleveland on May 20, 2025, presents results from EPA Method 1633 analysis of four water samples (T-1-2, M-1-2, B-1-2, and FB-1-2) collected on April 30, 2025. Three of the four samples—the main tank samples and middle samples—contained significant PFAS contamination, with detected analytes including perfluorooctanoic acid (PFOA) ranging from 1,200-1,300 ng/L, perfluorohexanesulfonic acid (PFHxS) ranging from 4,800-5,000 ng/L, and perfluorooctanesulfonic acid (PFOS) at approximately 38,000-43,000 ng/L. The field blank sample (FB-1-2) showed no detections across all analytes. The testing involved multiple analytical batches with dilution factors up to 10x and included comprehensive quality control measures with laboratory control samples, method blanks, and isotope dilution recovery assessments. The report notes several QC exceptions during analysis, including high-biased recoveries in continuing calibration verifications and isotope dilution analytes, though these were reported with appropriate qualifiers. Chain of custody documentation confirms proper sample handling and custody transfers throughout the analysis process. Eurofins Cleveland maintains Oregon NELAP accreditation (ID 4062, expiring 02-27-26) applicable to this testing.

Job Narrative 240-223402-1
Analytical test results meet all requirements of the associated regulatory program listed on the Accreditation/Certification Summary Page unless otherwise noted under the individual analysis. Data qualifiers and/or narrative comments are included to explain any exceptions, if applicable.
· Matrix QC may not be reported if insufficient sample is provided or site-specific QC samples were not submitted. In these situations, to demonstrate precision and accuracy at a batch level, a LCS/LCSD may be performed, unless otherwise specified in the method.
· Surrogate and/or isotope dilution analyte recoveries (if applicable) which are outside of the QC window are confirmed unless attributed to a dilution or otherwise noted in the narrative.
Regulated compliance samples (e.g. SDWA, NPDES) must comply with the associated agency requirements/permits.
Receipt The samples were received on 5/1/2025 8:00 AM. Unless otherwise noted below, the samples arrived in good condition, and, where required, properly preserved and on ice. The temperature of the cooler at receipt time was 1.5°C.
PFAS
Method 1633_Final: Sample(s) diluted to 25mL due to suspecting high AFFF readings. Laboratory proceeded with analysis.
T-1-2 (240-223402-1), M-1-2 (240-223402-2), B-1-2 (240-223402-3) and FB-1-2 (240-223402-4)
Method 1633_Final: The low level continuing calibration verification (CCVL) associated with batch 240-654821 recovered above the upper control limit for selected analytes. The samples associated with this CCV were non-detects for the affected analytes;
therefore, the data have been reported.
Method 1633_Final: The continuing calibration verification (CCV) associated with batch 240-654821 recovered above the upper control limit for selected analytes. The samples associated with this CCV were non-detects for the affected analytes; therefore, the data have been reported. The associated samples are:T-1-2 (240-223402-1), M-1-2 (240-223402-2), B-1-2 (240-223402-3) and
FB-1-2 (240-223402-4).
Method 1633_Final: The low level laboratory control sample (LLCS) for preparation batch 240-654497 and analytical batch 240-654821 recovered outside control limits for selected analytes. These analytes were biased high in the LLCS and were not detected in the associated samples; therefore, the data have been reported.
Method 1633_Final: Isotope Dilution Analyte (IDA) recovery is above the method recommended limit for the following sample:
FB-1-2 (240-223402-4). Quantitation by isotope dilution generally precludes any adverse effect on data quality due to elevated IDA recoveries.
Method 1633_Final: The continuing calibration verification (CCV) recovered outside of control limits for IDA compound(s) (flagged).
Section 14.3.3 of the finalized EPA 1633 states that the recovery of target analytes for the CCV must be within 70 - 130%, unless the analyte is not of concern for a given project. Since target analyte(s) associated with this IDA are within spec, data is reported.
The following sample is impacted: FB-1-2 (240-223402-4).
Method 1633_Final: Isotope Dilution Analyte (IDA) recovery is above the method recommended limit for the CCB associated with the following sample: FB-1-2 (240-223402-4). Quantitation by isotope dilution generally precludes any adverse effect on data quality due to elevated IDA recoveries.
Method 1633_Final: Sample(s) re-prepped due to 6:2 FTS failure in LLCS of previous batch. Sample(s) re-extracted at 10x dilution.
Laboratory proceeded with re-analysis.
T-1-2 (240-223402-1), M-1-2 (240-223402-2) and B-1-2 (240-223402-3)
No additional analytical or quality issues were noted, other than those described above or in the Definitions/ Glossary page.
