Statement of Work Revised 19 April 2024

1. Background

The 105th Stewart Air National Guard Base Building requires a fully operational vehicle maintenance shop. The maintenance shop has recently upgraded to utilize lifts for vehicle maintenance which has rendered floor pits unnecessary. The floor pits have been covered however this presents a tripping/falling hazard. Prior to the pits being covered they were filled in with crushed stone.

2. Scope

Contractor shall furnish all tools, parts, material, labor, supplies, equipment and incidentals required to properly perform work outlined below. All work shall be performed in a safe manner and shall be accomplished in accordance with all local, state, federal, and safety regulations to include any hazardous waste disposal if necessary.

3. Objectives

Demolition:

The contractor shall remove the cover plate guides and steel frame assemblies (see Figure 1) embedded in the surface slab by making a vertical saw cut a minimum of 2” outside of the assembly being demolished to a depth of 5”.

The contractor shall remove all concrete and lose material inside the vertical saw cut to a depth of 5 inches. The removal process shall not incorporate a pneumatic hammer larger than 30 lbs.

place the removed fill and spoils in a stockpile on base, location to be determined by the COR.

The contactor shall remove and discard all steel assemblies and mechanical devices from the pits off base. The contractor shall cap any penetrations to the pit to include but not limited to drains, supply conduits, weep holes etc.

Repair:

At the construction joint, where the existing slab meets the new concrete pour, the contractor shall insert fiberboard with expansion joint cap and place 5/8” diameter x 18” dowels no less than 18” on center a minimum of 3” below the surface. This placement shall be inspected by the COR prior to the concrete pour.

The contractor shall apply primer (as recommended by material manufacturer) to the entire surface of the area removed.

The contractor shall fill the pits with 4,000 PSI concrete to match the surrounding grade. The contractor shall utilize a concrete vibrator to consolidate concrete. The contractor shall provide a specification sheet to the COR prior to placement.

The contractor will be responsible for compressive strength testing of all concrete in accordance with ASTM C39. This shall include but is not limited to 7-day, 14-day and 28-day break results for each pour. It will be the contractor’s responsibility to ensure testing is performed by an ASTM certified lab and results shall be submitted to the KO for acceptance.

The contractor shall provide a concrete washout area in the storage yard adjacent to the project site. The washout area must be constructed in accordance with New York State Standards and Specifications for Erosion and Sediment Control section 2.24. The concrete washout area shall be utilized by all concrete trucks during the period of performance and the area shall be cleaned/restored to original condition prior to demobilization.

The contractor shall remove the expansion joint cap and seal the joints with a POL-resistant joint sealer. The contractor shall provide a specification sheet to the COR prior to placement.

The contractor shall ensure that all saw cut overcuts are thoroughly cleaned and completely filled with epoxy.

The contractor shall finish all concrete with a smooth finish and treat the surface with a non-skid surface coating to match existing adjacent floor.

4. General Requirements

1. The contractor shall provide all tools, materials, equipment, transportation, removal of debris and trash, supervision, and management (except as specified as Government Furnished) necessary to install, test, and repair facility safety items located at Stewart Air National Guard Base, Newburgh, New York.

a) Any waste of excess material resulting from any contract requirement shall be removed and disposed of by contractor at a proper disposal site off Stewart Air National Guard Station. Disposal, discharge, deposit, dumping, spilling, leaking, or placing of any materials, wastes, effluents, trash, garbage, oil, grease, paint, chemicals, etc. If any waste material is dumped in unauthorized areas, the contractor shall remove the material, restore the area to the condition of the adjacent undisturbed site at contractor’s expense.

b) The contractor shall comply with all Air Force, OSHA, and building codes requirements that are abiding by the Federal government, and State of New York.

c) Any additional work completed by the contractor without approval by the Contracting Officer shall be at the contractor expense.

d) The contractor shall submit material approval form to the contracting officer within 10 days of contract notice to proceed date. No materials shall be ordered without material approval of the contracting officer and the base civil engineer.

3. Government Furnished Property and Services. The government will provide water, sewer and electrical connections. Unsecure outdoor storage area at the base is available and must be requested through the project manager prior to utilizing. Secured storage is the responsibility of the contractor.

4. Quality Control. Contractor shall develop and maintain a quality program to ensure work is performed as described in this Statement of Work and in accordance with all local, state and federal rules.

5. Quality Assurance. The Government will maintain a Quality Assurance Program to ensure work is performed as described in this Statement of Work and in accordance with all local, state, and federal rules. The Project Manager will manage the Government’s QA program.

6. Hours of Operation. The hours of work shall be between 7:00 a.m. and 4:00 p.m. Monday through Friday unless otherwise prescribed by this statement of work. Work shall not be performed on Weekends and Federal Holidays without prior coordination from the Project Manager. NOTE: No work shall commence without prior coordination with the Base Facility Manager and the government project manager. Work within each building shall not commence without prior coordination of the applicable building manger, the contractor, and government project manager.

a) The contractor shall schedule and arrange work with the 105 CE building facility manager at least seven (7) working days before beginning work on-site. The contractor shall make every effort to minimize the interference and effect on the government mission.

b) PRIOR TO ANY WORK STARTING ON A FACILITY, A MANDATORY

WALK THROUGH SHALL BE SCHEDULED AND ACCOMPLISHED WITH

THE GOVERNMENT PROJECT MANAGER OR CONSTRUCTION

INSPECTOR FOR EXPLANATION AND REQUIREMENTS OF WORK

WITHIN EACH BUILDING.

7. Technical Competence and Ability. If requested by the Government, the Contractor shall provide proof of technical competence and experience in the area of expertise required by this

Statement of Work. The contractor shall be prepared to submit documents providing proof of successful competence such as training certificate or accreditations from recognized industry leaders. Contractors shall also be prepared to provide proof of successful project experience through references of former clients in related project types.

8. Dimensions and Quantity. All dimensions and quantities provided in the Statement of Work are estimates only. They are provided as a courtesy for contractors to estimate the magnitude of work or materials necessary to complete a task. They are not intended to be final measurements. The Contractor is responsible for verifying all dimensions and quantities in the field prior to submitting bids.

9. Warranty. The contractor shall guarantee all workmanship and materials covered under this statement of work for a period of no less than 1 year from the date of project acceptance. This contractor’s warranty shall be included with any manufacturer’s warranties that accompany materials or systems installed as part of this statement of work.

SUBMITTALS

All submittals will be transmitted with an AF Form 3000. When submitting on a finish, texture or other material characteristic where a photograph is unacceptable, the Contractor will provide a sample of the material for review and approval along with the product cut-sheet.

5. Delivery

All parts and materials shall be genuine replacement parts, as manufactured or recommended by the manufacturer. Only new standard parts shall be used in completed repairs. Performance shall take place at the 105th Airlift Wing, Stewart ANGB, Newburgh, NY.

6. Government Furnished Property: None

7. Security

Contractors should be aware that this is a Military Installation and that all access to this installation is granted by the 105 AW Base Defense Squadron. The U.S. government will not be held liable for any delays or breach of contract caused by refusal of the 105th AW Base Defense Squadron. If a gate pass is not granted to a contractor allowing the delivery of goods, execution of warranty support, or performance of services please contact the contracting officer.

A contractor’s badge will be issued once the contractor enters the main gate at Stewart Air National Guard Base. All employees are required to have the proper ID (social security card, driver’s license, state ID, immigration card, U.S. Passport, etc.). If they do not have the proper ID, they will be turned away. Vendors should be aware that Anti- Terrorism Force Protection Measures are applicable to this contract. Vendors will be briefed on these measures by Base Defense personnel if recurring or long term access is required to Stewart ANG Base. Otherwise you will be escorted at all times by a member of Stewart ANG Base.

8. Safety Requirements

The contractor shall comply with all Occupational Safety and Health Act (OSHA), federal, state, and local laws, regulations, and requirements regarding safety, as applicable to the services provided under this contract. In the event safety laws or regulations change during the term of this contract, the contractor is required to comply as such laws come into effect.
