STATEMENT OF WORK
FTQW 21-2001 Repair Roof Fall Protection
1.0 SCOPE
Base bid work will replace the building roof fall protection systems for B1171 and B3127 with a Sala Roofsafe Rail System, or similar. Option 1 work will include the repair and recertification of building roof fall protection systems for 1172, 1173, 3129, 3132, 3225, and 3225-1. Both the base bid and option will also include repair of roof damage caused by the fall protection system, which is primarily paint/finish damage. The end product will be fully functioning, certified fall protection systems which do not cause roof damage.
Contractor will furnish all labor, supplies, parts, transportation, tools, equipment, and facilities necessary to perform work required to restore operations in accordance with this Statement of Work and specifications.
2.0 Base Bid: INSTALLATION OF NEW SYSTEMS
Existing fall protection systems on B1171 and B3127 shall be demolished/removed. New Sala Roofsafe Rail, or similar, system shall be installed. The new system will provide the same level of roof access as the existing fall arrest systems, provide support for two maintenance workers at once, and use the same access points as the existing system.
Roof components damaged by the existing fall protection system shall be repaired. The primary roof issues observed are failed surface coatings, however may also include missing fasteners and damaged caps or flashing due to repeated contact between the cable and the roof. This is not an all-inclusive list of roof issues caused by the existing fall protection system and should be field verified by the contractor. Roof components damaged by the existing fall arrest system shall be properly repaired or replaced. Areas requiring resurfacing shall be prepared to remove corrosion, primed, and painted according to the roof panel manufacturer’s instructions/recommendations. Paint codes of existing roof colors are provided in Attachment 4. It is acknowledged that there may be some cosmetic variance between the touch up paint and the existing, weathered roof panels.
Work under the Base Bid shall comply with the Specification 11 24 00, which is attached.
2.1 Option 1: REPAIR OF EXISTING SYSTEMS
This work includes the repair and recertification of roof fall protection systems on Buildings 1172, 1173, 3129, 3132, 3225, and 3225-1. All buildings have standing seam metal roofs. Building locations are denoted on the map in Attachment 1.
Fall protection systems on the above facilities was accomplished under a single project in 2012, therefore share similar products/materials and design philosophy. The systems are primarily comprised of the Evolution Horizontal Lifeline system, with D-rings provided for smaller roof areas.
As-built drawings from this effort are included in Attachment 2, which should be field verified by the contractor.
The primary issues observed with the fall protection system are failed intermediate brackets. These brackets appear to have been damaged by the snow loads. Additionally, many of the mounting plates have loosened and have slid down the roof seam. This is not an all-inclusive list of issues and should be field verified by the contractor. The repaired system shall be designed and installed to properly accommodate snow and ice loads and prevent further damage to the roof. Any changes to the system shall be submitted as markups to the as-built drawings.

This appears to be caused by the use of improper intermediate brackets which do not hold the cable far enough from the roof, loss of cable tension, too few intermediate brackets, etc. This is not an all-inclusive list of roof issues caused by the existing fall protection system and should be field verified by the contractor. Roof components damaged by the existing fall arrest system shall be properly repaired or replaced. Areas requiring resurfacing shall be prepared to remove corrosion, primed, and painted according to the roof panel manufacturer’s instructions/recommendations. 3.0 GENERAL REQUIREMENTS
The contractor will be required to submit a safety plan/outline prior to beginning onsite work under this effort.
The contractor will be required to file a waste management plan for the contract, to include the types of waste which will be generated and the approximate quantities. Recycling is encouraged to the widest extent possible. Prior to contract closeout, weight tickets, or other similar documentation, must be provided showing final disposal of all waste materials.
All contractors performing work on Eielson AFB are required to complete onsite hazardous waste training provided by 354 CES/CEIEC. Contractor training must be completed two weeks prior to field work start date. Please contact 377-1659 to schedule a training for handling hazardous wastes, including aerosol can management. All costs associated with waste disposal are the responsibility of the contractor. Empty aerosol cans are the only waste accepted at the base hazardous waste facility for disposal. All hazardous wastes must be managed in accordance with the requirements for large quantity generators (40 CFR 26.17) under the resource conservation recovery act. Hazardous waste and non-regulated waste manifests must be signed by a designated POC prior to shipment off-base.
Please coordinate signature through the base hazardous waste facility (377-1668). Provide weight tickets for recyclables and materials that go to off-base landfills to solid waste program.
4.0 GOVERNMENT FURNISHED MATERIALS: None
5.0 INSTRUCTIONS TO THE CONTRACTOR:
5.1 Photography is restricted to personnel who have worked through the Contracting Officer to obtain required permissions.
5.2 Facilities will remain occupied and in use during the duration of the project.
Contractor may need to phase work in order to maintain adequate building access and parking availability. The contractor will be responsible for coordinating with the
Government prior to starting work in an area. The contractor will be responsible for establishing traffic control around work areas, as needed.
5.3 The contractor will have access to the work sites during normal duty hours of
Monday – Friday, 7:00 a.m. – 3:00 p.m, with exceptions of federal holidays and 168th Wing Alternate Work Schedule off days (Attachment 5). Any work outside of these hours must be coordinated with and approved by the Contracting Officer (CO) or Contracting Officer Representative (COR) a minimum of two business days in advance.
5.4 The period of performance runs for 75 days. Material lead times will be considered should the period of performance need to be extended.
5.5 The Contractor shall keep the work area clean of all debris on a daily basis or more often as required. The Contractor shall keep storage areas clean and orderly at all times. The Contractor shall dispose of combustible wastes at the end of each work day and under no circumstances allow them to accumulate. Hazardous materials must be handled according to the contract specifications.
5.6 Access and General Protection/Security Policy and Procedures. This standard language is for contractor employees with an area of performance within Government controlled installation, facility, or area. The contractor shall comply with all applicable installation/facility access and local security policies and procedures, which may be obtained from the Contracting Officer’s Representative (COR). The contractor shall also provide all information required for background checks to meet installation access requirements to be accomplished by the local installation’s Security Forces, Director of Emergency Services or local Security Office. The contractor shall ensure compliance with all personal identity verification requirements as directed by DoD, HAF and/or local policy. Should the Force Protection Condition (FPCON) change, the Government may require changes in contractor security matters or processes.
5.7 For Contractors that do not require CAC, but require access to a DoD Facility and/or
Installation. Contractor and all associated sub-contractors employees shall comply with adjudication standards and procedures using the National Crime Information Center Interstate Identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (AFI 31-101 and AFI 10-245), applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative), or, at OCONUS locations, in accordance with status of forces agreements and other theater regulations.
5.8 AT Level I Training. This standard language is for contractor employees with an area of performance within a Government controlled installation, facility or area. All contractor employees, to include subcontractor employees, requiring access to Government Installations, Facilities and Controlled Access areas shall complete AT Level I awareness training within 10 calendar days after contract start date or effective date of incorporation of this requirement into the contract, whichever is applicable. Certificates of completion for each affected contractor employee and subcontractor employee will be maintained by the COR or Antiterrorism Representative. AT level I Awareness Training is available through Joint Knowledge Online (http://jko.jten.mil/) by completing course JS-US007.

ariane.glover@us.af.mil; 907-377-1659): All contractors performing work on Eielson AFB are required to complete onsite hazardous waste training provided by 354 CES/CEIEC. Contractor training must be completed two weeks prior to field work start date and prior to receiving the Environmental Element signature on the Work Clearance Request (AF Form 103). Please contact 377-1659 to schedule a training for handling hazardous wastes, including aerosol cans management. All costs associated with waste disposal are the responsibility of the contractor. Empty aerosol cans are the only waste accepted at the base Hazardous Waste Facility for disposal. All hazardous waste must be managed in accordance with the requirements for large quantity generators (40 CFR 262.17) under the Resource Conservation Recovery Act. Hazardous waste and non-regulated waste manifests must be signed by a designated AF POC prior to shipment off-base. Please coordinate signature through the base Hazardous Waste Facility (377-1668) 10-days prior to planned transport.

• All POL and hazardous material leaks/spills must be reported to 911. Eielson Fire Dispatch will call the spill line (377-7745). Eielson Environmental will conduct all communication with the Alaska Department of Environmental Conservation and/or US Environmental Protection Agency and others, if required. Discovery of historical contamination must also be reported by calling the spill line.
16b. TOXICS ANALYSIS, COMMENTS, REQUIREMENTS (Ms. Ariane Glover; ariane.glover@us.af.mil; 907-377- 1659): Due to the age of Bldg 3127 (1954), lead based paint may be present; therefore, a lead-based paint survey will be required, if disturbing painted surfaces. Survey and sampling is the responsibility of the proponent. Please use appropriate PPE and follow disposal requirements, as needed. If disposing of surfaces with paint coatings, a TCLP lead sample (using Method 1311 in SW-846) will be required. Any results greater than or equal to 5 ppm must be handled and disposed of as hazardous waste. Note that an exemption exists for scrap metal recycling for metal exhibiting a characteristic waste. Scrap metal not recycled in accordance with the regulations is not excluded under 40 CFR 261.4(a)(13) or exempt under 261.6(a)(3)(ii).
Based on the age of the following facilities: 1171, 1172, 1173, 3129, 3132, 3225, and 3225-1, no known lead based paint concerns at those locations.

SUSPENDED MAINTENANCE AND FALL PROTECTION EQUIPMENT

PART 1 GENERAL
1.1 DESCRIPTION
Suspended maintenance and fall protection equipment including delegated design.
1.2 ADMINISTRATIVE REQUIREMENTS
a. Coordinate selection of roofing materials adjacent to anchorages. The roofing warranty shall be maintained. Flexible boots are required at roofing penetrations to allow for temporary deflection during testing and use.
b. Coordinate topcoat finishes applied over materials supplied under this specification section. Comply with paint and coating manufacturer's written recommendations for compatibility.
1.3 SUBMITTALS
Government approval is required for submittals with a "G" designation;
submittals not having a "G" designation are for Contractor Quality Control approval. Submittals with an "S" are for inclusion in the Sustainability eNotebook, in conformance with Section 01 33 29 SUSTAINABILITY REPORTING.

Manufacturer's Engineering Services
Manufacturer shall provide product information in appropriate file format to facilitate the design process and incorporate system solutions into project documents.
Provide project specific, scaled, engineered stamped shop drawings and calculations including layout, dimensions, loading and structural requirements.
Include analytical structural calculations of the connection and the supporting structure's capacity including service/fatigue design, testing, and ultimate/failure load combinations for each type of anchorage.
Roof plans with maintenance drop locations showing routing of temporary equipment supplied by the maintenance contractor such as ropes and counterweight beams.
Elevations Indicating Intermittent Stabilization Anchors

Log book including record shop drawings indicating the constructed condition with a completed initial testing log, yearly inspection log to be filled out by others, a 10-year retest log to be filled out by others, and an appendix containing the preconstruction submittals
Log book shall include the statement: Systems users to inspect all equipment prior to each use, including all visible attachment points, locks, and pins to ensure all equipment is in safe working order. All users shall be trained on proper use of the equipment, as well as knowing and complying with OSHA, ANSI, and other pertinent life safety regulations. All equipment shall be annually inspected by a qualified person and also re-certified within 10 years under direct supervision of a licensed engineer.

1.4 QUALITY ASSURANCE
a. Products shall meet or exceed OSHA and ASME A120.1 standards, and be tested and certified by professional engineers

1.5 DELIVERY, STORAGE, AND HANDLING
a. Provide packaging that will allow products to be stored outdoors or clearly mark packaging to indicate that products must be protected from the elements
b. Deliver and store products in manufacturer's unopened packaging bearing the brand name and manufacturer's identification until ready for installation. Handle materials to avoid damage
1.6 PROJECT CONDITIONS
a. Do not install products under environmental conditions outside manufacturer's recommended temperature limits
1.7 WARRANTY
a. Warranty: Provide manufacturer's standard limited warranty for parts for a term of 1 year on finish and structural capacity
PART 2 PRODUCTS
2.1 SYSTEM DESIGN REQUIREMENTS
a. Design and anchorage layout to comply with OSHA, ANSI, IWCA, applicable state regulatory requirements, and other project requirements as necessary.
b. Provide graphical design indicating areas of the building to be serviced. Provide complete coverage for the type of service required and service methods.
c. Fall Protection Anchorages:
1. Capable of sustaining a minimum ultimate load of 5,000 pound force in any direction the load may be applied for fall arrest.
d. Suspended Maintenance and Tieback Anchorages, Rigging Sleeves, Monorails, and Davits:
1. Capable of sustaining a 1,250 pound force service load in any direction the load may be applied under allowable stresses for material fatigue loading.
2. Capable of sustaining a 1,800 pound force impact service load in any direction the load may be applied for fall arrest loading.
3. Capable of sustaining a 5,000 pound force test load in any direction the load may be applied under the elastic limits of the materials.

e. Horizontal Lifelines:
1. Capable of sustaining a 1,800 pound force impact service load in any direction the load may be applied when two users are attached to the system with shock absorbing lanyards limiting the force applied to the system to 900 pounds force for each user.
2. Maintain a minimum factor of safety from service loads to elastic material limits of 2:1.
3. Capable of sustaining a minimum ultimate load of 5,000 pound force in any direction the load may be applied under the plastic limits of the materials.
4. All components of the system shall be directly specified and supplied including manufacturer, make, and model in the design. No replacement components or equals are allowed.
2.2 PRODUCT REQUIREMENTS
a. Anchors shall be completely free of sharp edges or abrasive surfaces.
b. Specifically deburr hoops after hot dip galvanizing process.
c. All exposed portions of carbon steel anchors shall be hot dipped galvanized or aluminum thermal sprayed.
d. Vent holes to accommodate the hot dip galvanizing process shall be permanently sealed.
1. Provide a minimum of 3/4" diameter attachment hoop. Attachment hoop shall be capable of sustaining 5,000 pound force in any direction the load may be applied under the plastic limits of the materials.
2. Inside radius shall be a minimum of 2 inches.
2.3 MATERIALS
a. Steel Shapes: ASTM A36.
b. W Shapes: ASTM A572, Grade 50.
c. HSS Tube and Pipe: ASTM A500, Grade B.
d. Aluminum: 6061 or 6063 with appropriate temper.
e. Welding Rods and Bare Electrodes: Per AWS requirements.
f. Galvanizing Repair Paint: High zinc dust content paint compatible with surfaces to be coated.
g. Exposed Fasteners: Galvanized steel.
h. Concealed Fasteners: Stainless steel.

2.4 FINISHES
a. Stainless steel, no applied finish.
b. Galvanized steel, no applied finish.
2.5 ACCESSORIES
a. Thermal Insulation: Load bearing thermal insulation separation material.
1. Thickness: 1/4 inch
2. Thickness: 1/2 inch
3. Thickness: 1 inch
PART 3 EXECUTION
3.1 EXAMINATION
a. Verify substrates match the recommendations of the manufacturer and the project specific shop drawings.
b. Verify bearing surfaces are even, plumb, and true. Uneven bearing surfaces may lead to loosening of anchorages during load testing and rejection of anchorage.
c. Do not begin installation until substrates have been properly prepared.
d. If substrate preparation is the responsibility of another installer, notify Architect of unsatisfactory preparation before proceeding.
3.2 INSTALLATION
a. Sequence and coordinate with other ongoing work, allowing appropriate notice to other trades and inspectors.
b. Install anchors and fasteners in strict accordance to manufacturer requirements.
c. Torque fasteners with a calibrated torque wrench in strict accordance to manufacturer requirements.
d. Fasteners shall be vibration proof through the use of permanent thread locking compound, deformed thread nuts, or damaged threads. Split ring lock washers are not acceptable without additional retention as noted.
e. Coordinate installation with the General Contractor to verify installation will result in a warrantable building envelope.
f. Prevent galvanic action and other forms of corrosion by insulating metals from direct contact with incompatible materials.
g. Prepare and touch up blemishes and carbon steel field welds with cold galvanizing compound.
