STATEMENT OF WORK
FTQW 21-2001 Repair Roof Fall Protection
1.0 SCOPE
Work will include the repair and recertification of building roof fall protection systems. Work will also include repair of roof damage caused by the fall protection system. The end product will be fully functioning, certified fall protection systems which do not cause roof damage.
Contractor will furnish all labor, supplies, parts, transportation, tools, equipment, and facilities necessary to perform work required to restore operations in accordance with this Statement of Work and specifications.
2.0 DESCRIPTION OF WORK
This work includes the repair and recertification of roof fall protection systems on Buildings 1171, 1172, 1173, 3127, 3129, 3132, 3225, and 3225-1. All buildings have standing seam metal roofs.
Building locations are denoted on the map in Attachment 1.
Fall protection systems on the above facilities was accomplished under a single project in 2012, therefore share similar products/materials and design philosophy. The systems are primarily comprised of the Evolution Horizontal Lifeline system, with D-rings provided for smaller roof areas.
As-built drawings from this effort are included in Attachment 2, which should be field verified by the contractor.
The primary issues observed with the fall protection system are failed intermediate brackets. These brackets appear to have been damaged by the snow loads. Additionally, many of the mounting plates have loosened and have slid down the roof seam. This is not an all-inclusive list of issues and should be field verified by the contractor. The repaired system shall be designed and installed to properly accommodate snow and ice loads and prevent further damage to the roof. Any changes to the system shall be submitted as markups to the as-built drawings.
The primary roof issues observed are failed surface coatings, missing fasteners, and damaged caps or flashing due to repeated contact between the cable and the roof. This appears to be caused by either the use of improper intermediate brackets which do not hold the cable far enough from the roof, loss of cable tension, too few intermediate brackets, etc. Photos from the Roof Condition Report are included in Attachment 3. This is not an all-inclusive list of issues and should be field verified by the contractor. Damaged roof components shall be properly repaired or replaced. Areas requiring resurfacing shall be prepared to remove corrosion, primed, and painted according to manufacturer’s instructions to maintain warranty. Paint codes of existing roof colors are provided in Attachment 4.
The contractor will be required to file a waste management plan for the contract, to include the types of waste which will be generated and the approximate quantities. Recycling is encouraged to the widest extent possible. Prior to contract closeout, weight tickets, or other similar documentation, must be provided showing final disposal of all waste materials.
The contractor will be required to submit a safety plan/outline prior to beginning onsite work under this effort.
All contractors performing work on Eielson AFB are required to complete onsite hazardous waste training provided by 354 CES/CEIEC. Contractor training must be completed two weeks prior to field work start date. Please contact 377-1659 to schedule a training for handling hazardous wastes, including aerosol can management. All costs associated with waste disposal are the responsibility of the contractor. Empty aerosol cans are the only waste accepted at the base hazardous waste facility for disposal. All hazardous wastes must be managed in accordance with the requirements for large quantity generators (40 CFR 26.17) under the resource conservation recovery act. Hazardous waste and non-regulated waste manifests must be signed by a designated POC prior to shipment off-base.
Please coordinate signature through the base hazardous waste facility (377-1668). Provide weight tickets for recyclables and materials that go to off-base landfills to solid waste program.
General work tasks include:
• Evaluate existing fall protection systems.
• Repair fall protection systems, to ensure replacement of failed components and ensure systems will not cause subsequent roof damage.
• Certify fall protection systems.
• Repair roof damage caused by fall protection systems.
3.0 GOVERNMENT FURNISHED MATERIALS: None
4.0 INSTRUCTIONS TO THE CONTRACTOR:
4.1 Photography is restricted to personnel who have worked through the Contracting Officer to obtain required permissions.
4.2 Facilities will remain occupied and in use during the duration of the project.
Contractor may need to phase work in order to maintain adequate building access and parking availability. The contractor will be responsible for coordinating with the Government prior to starting work in an area. The contractor will be responsible for establishing traffic control around work areas, as needed.
4.3 The contractor will have access to the work sites during normal duty hours of
Monday – Friday, 7:00 a.m. – 3:00 p.m, with exceptions of federal holidays and 168th Wing Alternate Work Schedule off days (Attachment 5). Any work outside of these hours must be coordinated with and approved by the Contracting Officer (CO) or Contracting Officer Representative (COR) a minimum of two business days in advance.
4.4 The period of performance runs for 75 days. Material lead times will be considered should the period of performance need to be extended.
4.5 The Contractor shall keep the work area clean of all debris on a daily basis or more often as required. The Contractor shall keep storage areas clean and orderly at all times. The Contractor shall dispose of combustible wastes at the end of each work day and under no circumstances allow them to accumulate. Hazardous materials must be handled according to the contract specifications.
4.6 Access and General Protection/Security Policy and Procedures. This standard language is for contractor employees with an area of performance within Government controlled installation, facility, or area. The contractor shall comply with all applicable installation/facility access and local security policies and procedures, which may be obtained from the Contracting Officer’s Representative (COR). The contractor shall also provide all information required for background checks to meet installation access requirements to be accomplished by the local installation’s Security Forces, Director of Emergency Services or local Security Office. The contractor shall ensure compliance with all personal identity verification requirements as directed by DoD, HAF and/or local policy. Should the Force Protection Condition (FPCON) change, the Government may require changes in contractor security matters or processes.
4.7 For Contractors that do not require CAC, but require access to a DoD Facility and/or
Installation. Contractor and all associated sub-contractors employees shall comply with adjudication standards and procedures using the National Crime Information Center Interstate Identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (AFI 31-101 and AFI 10-245), applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative), or, at OCONUS locations, in accordance with status of forces agreements and other theater regulations.
4.8 AT Level I Training. This standard language is for contractor employees with an area of performance within a Government controlled installation, facility or area. All contractor employees, to include subcontractor employees, requiring access to Government Installations, Facilities and Controlled Access areas shall complete AT Level I awareness training within 10 calendar days after contract start date or effective date of incorporation of this requirement into the contract, whichever is applicable. Certificates of completion for each affected contractor employee and subcontractor employee will be maintained by the COR or Antiterrorism Representative. AT level I Awareness Training is available through Joint Knowledge Online (http://jko.jten.mil/) by completing course JS-US007.

Building 1168, Aircraft Fabrication: This 21 year old inverted single-ply roof has vegetation growing in the paver joints and backed-out fasteners on the metal cap.
Building 1171, Fuels Maintenance: The fall arrest system cables are rubbing on the standing seams, several fasteners are backed-out and the expansion joint between sections A and B has a few holes on this 22 year old standing seam metal roof.
Building 1172, AGE Warm Storage: This standing seam metal roof is in good shape for being 23 years old. It only has broken plastic ridge end caps and a few backed-out fasteners for distresses.
Building 1173, Deicer Storage: This 23 year old standing seam metal roof is in good condition also. It has a few backed-out/ corroded fasteners and a little surface dete-rioration for distresses.
Building 1174, Fuel Station: All six penetrations are missing flashing boots on this 29 year old metal panel canopy.
Building 1176, Hangar: For only being 2 years old this single ply roof has several distresses. Numerous lumps under the membrane, some of which have created holes.
Also some of the base flashing has come loose and there is several areas of ponding.
Building 1180, LOX Storage: This 8 year old metal panel roof has some missing and misaligned foam panel closures.
Building 1191, AGE Parking: At 20 years old this modified bituminous roof is recom-mended for replacement. The substrate is exposed in several places, patches are de-teriorated and there are numerous ridges throughout the roof area.
Building 3127, Wing Headquarters: The 14 year old standing seam metal roof on this building is in great shape with only a few areas of surface deterioration.
Building 3128, Alert Garage: This 27 year old metal panel roof needs some panels replaced due to the fasteners corroding holes through some of the panels.
Building 3129, Operations: The metal panel sections of this roof are 17 & 23 years old and have backed-out/missing fasteners, surface deterioration, and the trees are rubbing on it on the north side. The new single-ply section has an abundance of abandoned cable laying on it and several lumps under the membrane. The older sec-tion has vegetation growing in the joints of the pavers that cover it.

Building 3133, Vehicle Maintenance: This 25 year old modified bituminous roof has a couple holes, a few improperly flashed penetrations, some loose base flashing and some debris for the surface granule deterioration.
Building 3180, Communications: The metal sections on this roof are distress free.
The single-ply sections have numerous distresses for only being 5 & 8 years old. The base flashing has several different distresses, the metal cap flashing also has several distresses and there is several areas of ponding due to the wrinkling of the membrane.
Building 3225, Civil Engineering: This 21 year old metal panel and single-ply roof is in good condition. The single-ply sections have some vegetation growing in the paver joints and the metal sections have a little surface deterioration.
Building 3325-1, CE Storage: The ridge cap on this 10 year old metal panel roof is deformed. Also a couple fasteners are corroded, the surface has a few scratches and a panel closure is misaligned.
Building 3345, Clinic: This 19 year old single-ply roof has some debris, a few lumps under the membrane and one base flashing reinforcement strip that is coming loose.

SW -- Low
1. Applied finish exhibits chalking, fading, or scratching over the entire roof sec-tion, but underlying corrosion is not apparent.
SW -- Medium
1. Base metal or applied finish exhibits chalking, fading, or scratching over the entire roof section, requiring a coating or surface treatment repair.
SW -- High
1. Base metal exhibits corrosion over the entire roof section, requiring a roof re-placement.
By definition, count the square feet of the entire area of the roof section.
Localized Surface Deterioration (SD) Degradation of factory applied finish on the panel, such as colored paint, clear coat, anodized finish; degradation of the field ap-plied coating; or metal corrosion.
SD -- Medium
1. Localized loss of applied finish or localized metal corrosion.
2. Metal panel finish exhibits degradation caused by grease, solvent, or oil drip-pings.
