About this file
This is a Past Performance Questionnaire (PPQ) Form template used to evaluate contractor performance on federal contracts within the past four years and current performance. The form is designed to be completed by contracting officials (government or private) to provide factual and accurate assessments of contractor capabilities for federal contract award decisions.
The PPQ Form contains five primary evaluation categories: Quality (assessing technical planning, engineering support, integration of specialties, and management of interfaces); Schedule (evaluating timeliness against milestones, delivery schedules, and administrative requirements); Cost Control (assessing forecasting, management, and control of contract costs, including cost growth or underrun analysis); Management (evaluating coordination, problem identification, corrective action plans, proposal submittals, and customer satisfaction); and Small Business Subcontracting (assessing compliance with FAR Subpart 19.7 requirements and maximizing opportunities for small businesses, disadvantaged businesses, women-owned businesses, HUBZone, veteran-owned, and service-disabled veteran-owned small businesses). Each category uses standardized adjectival ratings: Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or N/A. The form requires completion of contract identification information by the offeror, identification of the offeror's representative, and certification by the contracting official respondent, with the government reserving the right to verify all information provided.

Past Performance Questionnaire (PPQ) Form Attachment 0004 Amendment 01 The following questions pertain to the Contractor's record of past (within the past four years) and current performance for the contract identified below. The information that you provide as the Contracting (Government/Private) Official will be used in the awarding of a federal contract. Therefore, it is important that our information be as factual and accurate as possible. Please provide examples and/or explanations (use additional pages if necessary) sign the form, and return to individual identified in Part II: "Identification of Offerors Representative".
The following adjectival ratings shall be used in your reponse.
Exceptional: Performance meets contractual requirements and exceeds many to the Government’s/Customer's benefit. The contractual performance of the element or sub-element being evaluated was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
Very Good: Performance meets contractual requirements and exceeds some to the Government’s/Customer's benefit. The contractual performance of the element or sub-element being evaluated was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
Satisfactory: Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
Marginal: Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being evaluated reflects a serious problem for which the contractor has not yet identified corrective actions. The contractors proposed actions appear only marginally effective or were not fully implemented.
Unsatisfactory: Performance does not meet most contractual requirements, and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
