SOURCE QUALIFICATION STATEMENT

AMSC C: These parts require engineering source approval by the control activity to assure the integrity of the part. An alternative source must qualify according to the design control activity’s procedures as approved by the applicable government engineering activity, AFLCMC/WLSEAB (C-5M Super Galaxy Division)

SECTION A: ITEM IDENTIFICATION

SECTION B: JUSTIFICATION FOR ESTABLISHING A QUALIFICATION

REQUIREMENT

Overhaul of this complex design requires strict control of processes to ensure adherence to very close tolerances. Due to the unique application and Critical Safety Item designation of the visor door actuator, special equipment is required to test the final repaired item and ensure its flight worthiness. Brake, load, and functional testing are required to ensure the visor actuator will not fail in use.

Failure of the item would place the aircraft and crew in a hazardous situation. Failure of the visor actuator in use may lead to loss of life, damage to the aircraft, or destruction of cargo.

SECTION C: QUALIFICATION REQUIREMENTS

Source approval information: Listed below are the minimum requirements necessary to become a qualified bidder on the item listed above. Prospective contractors should report in accordance with MIL-HDBK-831.

a. Potential bidder shall provide a summary of the company’s capabilities, including a detailed factory layout, a list of the machinery, fixtures, tooling and test and inspection equipment.

b. Potential bidder shall demonstrate the ability and have experience in repair of similar critical aircraft components, which are currently in use on a large commercial or military transport aircraft and shall provide the following:

1. Contract Number

2. Procuring Agency

3. Contract Value

4. Number of assets shipped

c. Potential bidder shall provide a detailed quality plan implementing ISO 9002 (or equivalent).

d. Potential bidder shall perform more than 50% of the repair workload in-house.

e. Potential bidder shall conduct a complete overhaul of a unit and pass all testing in accordance with T.O. 16A1-14-40-3.

f. After qualification, the manufacturing plan processes, vendor sources, etc. shall not be changed without approval by the Government design activity, obtained through the contracting officer.

g. Once qualified, bidders shall be subject to first article testing if they have not previously manufactured this item within the last four years.

h. This qualification process shall be waived for the following companies who are already qualified for manufacture of NSN 1680-00-402-9812:

Thomas Instrument (CAGE 5H860)

i. The Air Force may not qualify potential bidders who have had quality problems when delivering similar or identical items to the government. Potential bidders who have produced similar items for the commercial market shall also be considered for qualification and shall provide commercial brochures and at least five (5) references with phone numbers

SECTION D: ESTIMATE OF THE TEST AND EVALUATION COSTS

No testing will be required for qualification. Development of a Source Approval package estimated cost is $0.

SECTION E: GOVERNMENT CONTRACT
