About this file

This is a Vendor Responsibility Questionnaire (VRQ) document issued by the State of Georgia for use in formal solicitations. The VRQ is required for all competitive procurements exceeding $100,000 and for contracts involving public health, infrastructure, or operational service delivery. The questionnaire serves as a pre-award compliance tool designed to proactively evaluate vendor integrity, financial stability, and adherence to applicable Georgia procurement laws, including compliance with O.C.G.A. § 50-5-50 (bidder responsibility), O.C.G.A. § 50-5-82 (prohibited sources), O.C.G.A. § 50-5-84 et seq. (scrutinized companies and foreign companies of concern), and O.C.G.A. § 13-10-91 (E-Verify requirements for public works). The document contains no specific product or service descriptions, quantities, response deadlines, site visits, bidders' conferences, award dates, or contract term information, as it is a standardized form rather than a solicitation notice.

The VRQ requires vendors to complete five sections addressing general business identification, legal and regulatory compliance, financial and operational stability, tax compliance, and certification under oath. Vendors must disclose any debarments, environmental or safety violations, procurement fraud convictions, OSHA citations, ongoing investigations, contract performance failures, and tax liabilities within specified lookback periods. All responses must be truthful and complete, with "Yes" answers requiring detailed written explanations and supporting documentation. The final page must be notarized with a valid notary commission expiration date. No pricing information, set-asides, incumbent contractor data, or funding amounts are contained in this form, as it is a qualification and responsibility assessment tool rather than a pricing or contract award document.
