About this file

The Vendor Responsibility Questionnaire (VRQ) is a procurement document issued by the City of Sandy Springs, Georgia, designed to comprehensively evaluate vendor eligibility and integrity for formal solicitations exceeding $100,000 or involving public health, infrastructure, or operational service delivery. The document requires vendors to disclose detailed information about their legal, regulatory, financial, and operational status, with mandatory compliance checks across multiple sections including legal history, environmental violations, OSHA citations, procurement practices, financial stability, and tax compliance.

The questionnaire serves as a proactive risk mitigation tool, requiring vendors to provide transparent disclosures about potential legal, financial, or operational issues that could impact their ability to successfully complete a government contract. Key compliance areas include verifying good standing with the Georgia Department of Revenue, confirming financial capacity to perform contracts, maintaining required insurance, and affirming the ability to obtain performance bonds. Vendors must complete all sections truthfully, with potential disqualification for failure to disclose relevant information, and the final document must be notarized to validate the information provided.
