About this file

This is a Vendor Questions Worksheet for the State of Michigan used to collect bidder information and attestations in response to RFP 260000002641. The document serves as a comprehensive questionnaire requiring bidders to provide detailed responses across multiple categories including company background, experience, qualifications, and compliance certifications. The worksheet does not specify particular products or services, project scope details, quantities, response deadlines, award dates, or contract terms within its content, as it functions as a standardized information-gathering tool rather than a solicitation document with specific project parameters.

The worksheet requires bidders to affirm agreement with attached contract terms and insurance requirements (Schedule C) and to disclose relevant experience from the past five years supporting their capability to manage similar contracts. Bidders must also attest to compliance with numerous Michigan and federal requirements, including labor law compliance, environmental regulations, prevailing wage obligations under Public Act 10 of 2023, proper employee classification, non-use of forced or child labor, Michigan business tax filing obligations, Iran-linked business restrictions, clean corporate citizen status, state tax payment, and authorization for state verification of provided information. The worksheet includes questions regarding state of Michigan economic impact, including current and projected Michigan-based employment and wage information, as well as disclosure of any debarment, suspension, litigation, contract defaults, or violations within the past five years. Bidders must also confirm their status with the Michigan Voluntary Protection Program and indicate whether they qualify as disabled veteran-owned businesses.
