The document requires an authorized company representative to certify under penalty of perjury compliance with prohibitions against using physical force, restraining or confining persons, exploiting labor through debt, withholding identification documents, threatening financial harm, using fraud or deceit to exploit persons, or providing controlled substances for exploitation purposes.
All vendors submitting proposals for the occupational health services contract must complete and submit this affidavit as a mandatory compliance requirement. The affidavit must be signed by an officer or authorized representative of the contracting company and submitted with the proposal due on June 19, 2025, at 3:00 PM.
