About this file
Summary
This is a Vendor Affidavit document from Hillsborough County Public Schools (HCPS) in Florida regarding compliance with human trafficking prevention requirements under Florida Statutes Section 787.06(13). The affidavit is required for all nongovernmental entities executing, renewing, or extending contracts with HCPS and must be signed by an authorized officer or representative under penalty of perjury. The document pertains to the Hood Cleaning Services solicitation (ITQ 25509-ITQ-MST), which seeks bids for professional cleaning services for concessional and culinary stand exhaust hoods across 235 PreK-12 schools and specialty centers in Tampa, Florida. HCPS estimates approximately 70 annual hood cleanings will be required. Vendors must submit electronic quotes through the VendorLink system by November 13, 2025, at 4:30 PM, with the final deadline for submitting questions on November 6, 2025. The initial contract term begins upon Board approval and may automatically renew for up to four additional one-year periods, contingent on budget appropriations, contractor performance, and mutual agreement.
The vendor affidavit specifically requires contractors to certify under penalty of perjury that their company does not use coercion for labor or services, including threats of physical force, unlawful restraint or confinement, exploitative credit practices, document confiscation, financial harm, fraud, or controlled substance provision. This certification is a mandatory compliance requirement for contract eligibility. Additional requirements include submission of a vendor registration form, background checks, drug-free workplace compliance, proof of insurance, and an affidavit certifying no ownership or principal business location in designated foreign countries including China, Russia, Iran, North Korea, Cuba, Venezuela, and Syria. The solicitation encourages participation from diverse businesses, including small businesses, service-disabled veteran-owned, and minority-owned enterprises. Evaluation will focus on the Quote Pricing Sheet with contractors allowed a maximum 15% markup on materials and required to maintain firm pricing for at least one year.
