About this file
This is a Veterans Business Program (VBP) Goal Waiver Request and Review Form (Version 26.1) issued by the State of Illinois, administered through the Chief Procurement Office under the Illinois Procurement Code. The form is designed for vendors bidding on state contracts to request and document waivers from the state's requirement to award not less than 3 percent of total contract dollars to small businesses owned and controlled by veterans and service-disabled veterans as established under 30 ILCS 500/45-57(a). The form applies to the DNR Carlyle Lake Stabilize Riverside Embankment project across three locations in Illinois. Vendors submitting bids must complete this form in detail if they cannot meet the VBP utilization goal, with failure to do so potentially rendering bids non-responsive.
The waiver request process requires prime vendors to demonstrate good faith efforts across multiple dimensions: documenting rejection criteria for certified vendors based on sound justification, providing evidence of sufficient outreach time and communication to eligible certified vendors, demonstrating good faith negotiations with VBP-certified firms including specific names and contact information, and showing efforts to break work into economically feasible units to facilitate certified vendor participation. The form requires vendor authorization and submission to the purchasing entity identified in the solicitation. The purchasing entity then reviews and makes a recommendation, with final determination made by the CEI Deputy Director. All completed forms with required signatures must be submitted to CEI's VBP Compliance at Jacob.Perez@Illinois.gov and included in the procurement file.
