ACCOMPANYING AMENDMENT– VA69D-17-B -1592 00001

RENOVATE 3A/3C

WILLIAM S. MIDDLETON VA HOSPITAL, MADISON, WI

I. THE FOLLOWING CHANGES ARE TO BE MADE ON PROJECT SPECIFICATIONS:

1. Insert the following additional new Specification Section:

0. 11 73 00 Ceiling Mounted Patient Lift System.

II. THE FOLLOWING QUESTIONS AND ANSWERS / CLARIFICATIONS ARE THE RESULT OF BIDDERS REQUEST(S) FOR INFORMATION:

Q1. Are as built drawings of the original construction available? This would be helpful in determining the wall types needed to be demolished.

A1. Original as builds can be made available upon request, typical wall type construction is included with the amendment.

Q2. On G101 and area is shown to be the Dumpster Staging Area for the project. That area will be taken up for the Cancer Center project that is currently under contract. Please identify an alternate area as this space won’t be available during construction.

A2. Dumpster areas will be designated in proximity to location identified.

Q3. The stairs and platform shown on A112 and in the structural drawings will need to go through an existing roof. Is this roof currently under warranty? If so please detail any additional work needed to maintain existing warranties.

A3. Stairs will not penetrate through roof structure. Roof is EPDM and any alterations should comply with construction standards.

Q4. Please provide the construction schedule for the project referenced on A112 that needs to be coordinated with for access above the hard ceiling. If the phasing doesn’t overlap for installation who is responsible for any additional schedule costs and impact, i.e. will that contractor keep the ceiling open for additional time fi needed for this project? If the of contractor are delayed who covers that cost? If this contractor needs to accelerate to meet their time who covers that cost?

A4. Base bids on 3C phase of the project occurring concurrent with renovation on 2C, however bidder contractor is responsible for routing as per plans and getting access to the space required as a part of this project.

Q5. Alternate Bid Item #1: Furnish and install all work as described in the All Inclusive Base Bid except: OMIT complete removal and replacement of FCU 3A-3, FCU 3A-4, FCU 3A-5, FCU, 3A-6, FCU 3A-7, FCU 3A-8, FCU 3A-9, FCU 3A-10, FCU 3A-11, FCU 3A-12, FCU 3A-13, FCU 3A-14, FCU 3A-15, FCU 3A-16, FCU 3A-17, FCU 3A-18, FCU 3A-19, FCU 3A-20, FCU 3A-21, FCU 3A-22, FCU 3A-23, and FCU 3A-24, as indicated. In lieu of complete replacement of fan coil units, and associated valves, reuse fan coil units and replace existing pneumatic control valves with new DDC control valve. Question regarding this alternate: All the FCU's are shown as fed from new panels, so if the existing units stay, will new electrical wiring still be required to the existing FCU’s?

A5. Yes, new wiring will be needed to existing FCU’s as all new electrical wiring/panels are to be added.

Q6. For temporary doors does the VA required card reader security pads and/or trilogy brand lever locksets with Sargent cores?

A6. Contractor shall supply and install Trilogy brand keypad locksets with VA provided Sargent/Medeco cores at all construction barrier doors.

Q7. Contractor is to salvage light fixtures. Please confirm that this is only for D-Wing.

A7. As note states on AD-131, existing toilet room D3205 ACT ceiling and grid to be removed for installation of new duct work. Existing light fixtures and ceiling mounted devices shall be salvaged and re-installed in new ceiling. Note is for D-Wing toilet D3205 only.

Q8. Does the VA intend on utilizing submittal exchange on this project? Will this software be a cost of the contractor or included in the specifications per amendment? This is a $6,995 fixed cost by Oracle for this size project.

A8. The VA will provide contractor access to Submittal Exchange for electronic submittal processing on this project.

Q9. Division 04 is not utilized on this project; however, there are penetrations and anchoring for rooftop unit steel on this project. Would VA like to advise on these parapet wall tie-ins if there is masonry anchoring or patching.

A9. Specification Sections 03 30 53 and 05 50 00 covers anchoring.

Q10. Bolded are the phasing requirements for the project. Are there any phasing constraints or milestones on this project due to hospital occupancy needs within any of the 6 phases? Please reconfirm that A and C wing cannot be in a start-to-start relationship. Please confirm if C wing must gain beneficial occupancy prior to starting the demo of A wing.

A10. Phasing is labeled phases 1-6 to be executed accordingly as to minimize current working clinic disruptions. Phases are sequential and not permitted to be working on simultaneously, ie Phase 1 shall complete prior to Phase 2 commencing, Phase 2 shall complete prior to commencing Phase 3.

Q11. Please confirm if non-construction personnel will have egress of any of the three phases shown for A and C wing. Particularly movement within C wing renovation with occupants moving from A wing to potentially column line P/Q or to Column line 14 moving east to west through A wing. Essentially, will the corridors remain public?

A11. Refer to phasing requirements. Access/egress through non-construction areas is required for C-wing and Awing, as indicated.

Q12. EMC has a published Asbestos Pre-Renovation Inspection List (pdf 155/733 in Vol 1 Spec). Typically the VA spec takes precedence over dwgs. Which pipe insulation listing takes precedence for this job? Spec pdf page 155 or H-100.

A12. Specifications take precedence.

Q13. Is the demountable partition a salvaged item for the VA?.

A13. No. Remove and dispose of partition.

Q14. Please help with the BOD for artwork. We cannot find any on the elevations in the A400's dwgs.

A14. Artwork to be VA provided, VA installed.

Q15. Please confirm that the VA is responsible for CxA where GC is CxR.

A15. VA will hire the Commissioning Agent.

Q16. Please confirm that the DIV 23 controls contractor cannot also be the DIV 25 contractor.

A16. Correct, they may not be the same.

Q17. Please confirm that the Tridium BAS is sole source at the hospital.

A17. Tridium is an open platform that multiple contractors may bid on. Currently there is not a sole source to a specific contractor. The current specification lists a specific contractor or equal. Equivalent requirements lists are in the specifications.

Q18. What is the anticipated start date for this project?

A18. It is anticipated that award and notice to proceed will be issued within 90 days of bid date.

Q19. Drawing I-402 Room Finish Legend Lists 3 form panels with style and color TBD, have these been decided as these selections determine pricing.

A19. Color selections to be made during submittals.

Q20. Alternate Bid #2 states to delete all labor and materials associated with provisions and installation of patient lift systems in exam rooms. Please provide a spec section for these patient lifts or manufacturer to be used.

A20. Specification section 11 73 00 is included with the amendment.

Q21. The mechanical plans (MPD-101 thru MPD-104 & MP-101 thru MP-104) all seem to state that the base bid is to replace the controls on the existing fan coil units as the base bid with an alternate for replacing the fan coil unit complete with controls, however, the “Bid Schedule” or bid form states that the base bid is to replace the fan coil units complete with controls with an alternate to replace only the controls on the existing fan coil units. Please clarify

A21. As stated, In lieu of “COMPLETE” replacement of fan coil units and associated valves, “REUSE” fan coil units and “REPLACE” existing pneumatic control valves with “NEW” DDC control valve.
