About this file

This is a VA CWM Utility Shutdown Request form and accompanying instructions document used to coordinate the shutdown of utilities at Department of Veterans Affairs facilities during maintenance or construction work.

The form requires contractors to provide detailed information including project number, company name, shutdown point of contact and phone number, requested shutdown date(s) and times, specific locations of shutdown and work, location of Lock Out Tag Out (LOTO) equipment, competent person(s) performing the work, required personal protective equipment (PPE), reason for shutdown, and complete description of work. Contractors must identify all buildings and areas impacted by the shutdown, including downstream and upstream systems affected, and specify which services will be shut down versus merely affected (Steam, HVAC, Normal Power, Domestic Cold Water, Domestic Hot Water, Heating Hot Water, Chilled Water, Sanitary System, Nurse Call, Fire Alarm, Sprinkler System, Emergency Power, IT, or Other). Minor shutdowns require seven calendar days' written notification, while major shutdowns affecting Medical Center operations require 21 calendar days' notice. All Fire Alarm and Sprinkler shutdowns must be coordinated with the VA Fire Alarm Technician, and all required LOTO equipment and mandatory PPE must be on site or the shutdown will be cancelled. The VA Contracting Officer's Representative (COR) and Operations & Maintenance Shop must review and approve the request. The document outlines specific responsibilities for contractors, the VA COR, and M&O shops, including system investigation, impact assessment, temporary measure implementation, and coordination with affected supervisors and staff to maintain Medical Center operations.
