About this file
This document is a VA CWM Utility Shutdown Request form and instructions for managing utility interruptions at VA Medical Center facilities.
The form requires contractors to submit shutdown requests with specific information including project number, company name, shutdown point of contact and phone number, requested shutdown date(s) and times, locations of shutdown and work, LOTO (Lock Out Tag Out) location, competent person(s) performing the work, required PPE, reason for shutdown, work description, and all impacted buildings and areas including downstream and upstream systems. Contractors must identify which utilities will be shut down and which will be affected, selecting from Steam, HVAC, Normal Power, Domestic Cold Water, Domestic Hot Water, Heating Hot Water, Chilled Water, Sanitary System, Nurse Call, Fire Alarm, Sprinkler System, Emergency Power, IT, and Other. Minor shutdowns require 7 calendar days' written notification, while major shutdowns affecting Medical Center operations require 21 calendar days' notification. All Fire Alarm and Sprinkler shutdowns must be coordinated with the VA Fire Alarm Technician. Contractors must have all required LOTO equipment and mandatory PPE on site or the shutdown will be cancelled.
The instructions delineate responsibilities for contractors, VA Contracting Officer Representatives (CORs), and M&O (Maintenance and Operations) Shops. VA M&O shops require 72-hour notice from VA CORs for all shutdowns. Contractors must provide panel photos and schedules with the request and involve the COR early for system investigation. The COR reviews accuracy, notifies relevant parties to prevent conflicting shutdowns, confirms affected systems and critical functions, communicates with supervisors and staff, ensures temporary measures maintain operations, and provides final notification upon approval. M&O Shops review accuracy, report known issues, specify required support, and return completed forms to the COR. A fully executed copy of the request form is required before shutdown can proceed.
