About this file
This is an Iran Economic Sanctions Act Certification form for the EUP Erate Consortium, with the Ishpeming-Negaunee-Ishpeming Intermediate School District (EUPISD) serving as fiduciary. The certification applies to bidders submitting proposals for Erate Category 1 and Category 2 services. The bidder or authorized representative must certify that they have personal knowledge of the matters described and are familiar with the Iran Economic Sanctions Act (MCL 129.311, et seq.), and that the bidding entity is not an Iran-linked business as defined by the Act.
Bidders must understand that submission of a false certification carries significant penalties, including potential contract termination, disqualification from bidding for three years, and civil penalties of $250,000 or twice the bid amount, whichever is greater, plus related investigation and legal costs. The certification requires a signature, printed name, and date from the authorized bidder representative or individual with binding authority to submit the bid.
