Uniform Consultant Questionnaire - City of Benson Engineering Services

This is a Uniform Consultant Questionnaire evaluation document used by the City of Benson, Arizona, to screen and qualify engineering firms for on-call design and review services. The City of Benson is conducting a competitive one-step procurement process to award multiple qualified contractors for miscellaneous engineering design and review services under Arizona Revised Statutes Section 34-603. Individual job orders under the resulting contract are not expected to exceed $150,000, with total annual assignments estimated at $300,000. Services will be requested on an as-needed basis, and the contract is neither exclusive nor a guarantee that contractor services will be required. The questionnaire requires firms to submit detailed responses addressing their available resources, experience on similar projects, firm qualifications, team experience and qualifications, and past performance history with the City.

The evaluation criteria are ranked in order of importance as follows: available resources to complete the project including organizational charts; firm experience on similar projects with particular emphasis on City of Benson work; qualifications of the firm including management capabilities and quality control procedures; experience and qualifications of the project team including the project manager and project engineer; and past performance assessed by the evaluation committee using information from City departments, project managers, outside agencies, and consultant-provided references. Firms must provide an organizational chart showing assigned personnel and specific project tasks, list ongoing projects and explain capacity, identify the schedule manager, provide references from recent clients, describe internal quality and cost control measures, and submit lists of all applicable City of Benson contracts in progress and completed within the preceding five years, including contract numbers, dates of award, using department, and total contract fees.
