Coats are to be marked with supplier provided identification (ID) tags referencing the location of the respective pickup and delivery location the garment belongs. A sample ID tag shall be included with your response. (photo is acceptable) The awarded proposer will scan each laboratory coat each time the garment(s) are turned in for service. Every account shall receive a printed copy of a weekly report accompanying the clean returned coats, to be submitted with invoicing. This report will include the coats that were picked up and returned, the pick-up location, the name on the laboratory coat, (or “House Inventory”) and quantity scanned in and out for that account. Awarded vendor shall agree to make any reasonable changes to this report to meet UConn Health’s requirements. Each department should be invoiced separately, on a weekly basis. The awarded proposer will provide a monthly statement to each account for use in reconciling the weekly invoices and for use in processing payments. All invoices must include the purchase order number. Provide a sample report and invoice with your response.

Awarded vendor must (a) notify UCH business unit (email is acceptable) within twenty-four (24) hours if any garment submitted for services is damaged. Damage includes, but is not limited to g., visible wear, soiling, structural defects, or defects discovered after production.
