Notes:

1) Code reference is current edition at the time of contract award.

2) If an item is the responsibility of CMOP it shall be annotated at time of contract award.

3) Contractor is responsible for confirming frequencies.

4) Contractor is responsible for submitting documentation to CMOP on a monthly basis in accordance with the contract. The documentation shall have the name of the activity, date of activity, inventory, required frequency, name and contact info of person who performed activity, NFPA standard referenced, and results. Any failures shall be tracked until resolution with a follow up report to CMOP for each activity.
