About this file

This is a Title VI Training Response Form submitted to the New York State Department of Health in response to a Request for Quote for the development and delivery of training on Title VI of the Civil Rights Act of 1964 Policies, Procedures, and Investigations. The training is intended for staff employed by the state governmental health agency and must address the Department's Federal Civil Rights Nondiscrimination Program requirements. The vendor response form requires submission of two attachments: Attachment B (Quote) and Attachment C (Vendor Information). The Quote section is structured in three main categories: Training Delivery, New Curriculum Development, and Miscellaneous costs. Training Delivery options include virtual live webinars of varying durations (up to 2 hours, 2-4 hours, half day, and full day) and online recorded training, with vendors required to specify quantities and unit prices. New Curriculum Development costs are itemized by webinar duration type, and Miscellaneous costs include platform fees, frequently asked questions documents, and follow-up resource documents, with optional fields for additional costs.

The Vendor Information section requires respondents to provide five substantive narrative responses addressing company qualifications, instructor credentials, years of operation, staff biographies, client references, and detailed training structure recommendations. Vendor instructors must demonstrate minimum qualifications including at least three years of professional experience developing and delivering live and recorded training curriculum, specialized knowledge of Title VI and other civil rights laws as applied within state governmental agencies, ability to provide detailed and tailored instruction on Title VI compliance policies and procedures, and familiarity with federal and state civil rights regulations. The response form specifies word limits for each narrative section (150-500 words depending on the question) and requires that proposed training length and structure align with the associated costs submitted in the pricing quote section.
