If “No” was the response to the previous question, please briefly describe the type of work effort or project that the contractor did perform?

1. Please describe the contractor’s performance as it relates to the Quality/Performance under your contract, delivery or task order:

2. Please describe how the contractor controlled costs and remained on budget under your contract, delivery or task order:

3. Please describe if the contractor met all the schedules, deliverables and/or reporting requirements for your contract, delivery or task order:

4. If your contract, delivery or task order required maintenance of a Major IT System, peripheral equipment, or COTS software licenses, please answer the following three questions:

a. Describe the contractor’s performance with maintenance of major IT systems (i.e., mainframes), peripheral equipment, or COTS software licenses: ________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________

b. Describe the contractor’s ability to work with multiple stakeholders: ________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________

c. Describe the contractor’s performance maintaining system availability: ________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________

5. If your contract, delivery or task order required maintenance of Commercial-off-the-shelf (COTS) software licenses for a Major IT System, such as for one or more mainframe computers, please answer the following four questions. (COTS software license maintenance is also referred to as management of software license subscription charges and payments, and does not involve programming support or other customization of the COTS software):

a. Describe the contractor’s performance with managing COTS software license maintenance issues, such as ensuring that software keys/codes are made available when needed (and replacements are provided when older software keys/codes expire); the contractor informs the customer whenever new software versions or upgrades are released; and the software license types are the correct ones for the customer’s IT system(s): ________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________

b. Describe the contractor’s ability to work simultaneously, with multiple COTS software providers, if applicable, such as when the customer’s major IT system is upgraded or replaced. For example, is the contractor able to manage software processing capacity limit increases, when the customer adds processors to an existing mainframe system? Does the contractor provide periodic status reports on the customer’s software licenses?: ________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________

c. Describe the contractor’s performance when software license support is needed to conduct disaster recovery testing and/or performance (or pre-production) testing. In the event that the customer encountered an actual disaster situation, was the contractor able to help get the software licenses running on the customer’s disaster recovery system? If the customer had an unexpected problem with the COTS software product(s), was the contractor able to help resolve the situation?: ________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________

d. Describe the contractor’s knowledge and expertise with various COTS software licenses in use for the customer’s IT system, especially if this is in the IBM mainframe environment. For instance, is the contractor familiar with licenses based on Gartner MIPS ratings, IBM MSUs, system model number, logical partition (LPAR), and other license types?

If additional sheets are needed for any answers, please attach.
