About this file

This document is a Taxpayer Identification Number (TIN) form for the State of Illinois. The form is used to collect the name, business name, and taxpayer identification number (Social Security Number or Employer Identification Number) of the authorized representative. The form also requires the legal status of the entity, indicating whether it is an individual, sole proprietor, partnership, corporation, or other type of organization. The completed form must be signed and dated by the authorized representative.

The form does not contain information about a specific state or local contract opportunity. It is a standard administrative form used to collect taxpayer identification information, which is likely required as part of the bidding or contracting process for state and local government agencies in Illinois.
