About this file
This is a compliance assurance document for the Drug-Free Communities (DFC) Support Program that establishes the ten-year funding limit restriction under the Drug-Free Communities Act of 1997. The document requires the Legal Applicant to certify that the applicant coalition has not received more than ten years of DFC funding and has not violated the stated restrictions. The document outlines two categories of restrictions: restrictions on Legal Applicants serving as Fiscal Agents, which prohibits them from receiving DFC funds on behalf of coalitions that have already received ten years of funding; and restrictions on 501(c)(3) coalitions, which limits such coalitions to a maximum of ten years of DFC funding regardless of the number of Legal Applicants involved and prohibits further DFC funding once the ten-year threshold is reached.
The Legal Applicant must sign and date the assurance form, providing the coalition name and applicant contact information. The certification affirms compliance with the ten-year funding limit and that no restrictions have been violated. If additional information is needed to verify the coalition's status as new, unique, and distinct, the Legal Applicant will be notified. This assurance is a required component of applications for the DFC Support Program, which is administered by the Department of Health and Human Services Centers for Disease Control and Prevention (CDC) and the Executive Office of the President's Office of National Drug Control Policy (ONDCP).
