About this file

This is a sector representatives table template that applicants must complete as part of the Drug-Free Communities (DFC) Support Program Competing Continuation grant application for Fiscal Year 2026. The template requires applicants to identify and document representatives from 12 distinct community sectors, including youth, parents, business, media, schools, youth-serving organizations, religious or fraternal organizations, law enforcement, civic/volunteer groups, healthcare professionals or organizations, state/local/tribal government agencies with substance abuse expertise, and other organizations involved in substance abuse reduction. For each sector representative, applicants must provide the individual's name, their organization affiliation, and a detailed selection rationale that demonstrates their qualifications to represent that sector according to definitions provided in the Notice of Funding Opportunity (NOFO).

The DFC Support Program, administered by the Department of Health and Human Services Centers for Disease Control and Prevention (CDC) in partnership with the Office of National Drug Control Policy (ONDCP), provides grants to community coalitions working to prevent and reduce substance use among youth aged 18 and younger. This competing continuation opportunity funds coalitions that have completed their initial five-year funding cycle or experienced a funding lapse. Grant awards typically range from $250,000 to $500,000 annually with performance periods up to five years. Applicant coalitions must demonstrate at least six months of prior collaboration on substance abuse reduction, maintain nonprofit or government status, ensure substantial volunteer involvement, provide 100-150 percent cash or in-kind match, and commit to program evaluation.
