IV. PROJECT DESCRIPTION

1.0 Site-based considerations:

This well is currently covered with tin to keep the landowner’s grandchildren from falling in the well. The 6 5/8” casing is rotted off at the surface and has a jagged edge. This well has been known to discharge oil in the past. The site is located along a drainage behind a residence. A section of the original lease road is intact and will be upgraded to access the site.

2.0 Well-specific considerations:

The Office of Oil and Gas does have a well record for the subject well API# 47-017-00154. The record well shows total depth (TD) is 2882’. The well record indicates that the well has 1803’ of

6 5/8” casing left in well. Cemented casing locations are unknown. The record shows freshwater at 30’, saltwater at 1265’, gas at 1897-1909’ and no mention of coal. The well was shot @ 2585’, 2578’, 1912’ and 1898’.

V. PROJECT WORK PLAN

Contract work may require site remediation of the well site prior to the commencement of plugging operations.

Project will commence, generally, as follows:

1. Pre-mobilization site remediation, if necessary,

2. Site preparation,

3. Connect rig to well,

4. Clean out well to total depth,

5. Set prescribed plugs and monument,

6. Reclaim site including removal of stream crossings, and

7. Submit invoice to DEP.

Site Preparation Plan

Two hundred (200) feet of silt fence will be installed along the road to the site to isolate the disturbed area. The area around the well (50’ x 100’) shall be cleared of all brush and trees and will be leveled and smoothed. All trees and brush can be stacked and left on location. The existing lease road will need to be widened and stoned to accommodate plugging equipment. Culvert and stone will likely be needed to ready the site to accommodate the rig and equipment. A minimum

24” cellar shall be installed around the well head.

Proposed Plugging Work Plan:

1. Notify Project Coordinator and District Oil & Gas Inspector within 24 hours of plugging commencement.

2. Move in and rig up to existing 6 5/8” casing.

3. Measure in hole to check TD.

4. If TD cannot be reached, begin cleaning out well bore to TD.

5. TIH to TD with tubing, and spot 6% gel and set a minimum of 100’ cement plug from

2882’-2782’. Trip out hole (TOH), wait on cement (WOC).

6. Tag top of plug. Add cement if needed.

7. TIH with tubing, and spot 6% gel from 2782’ to 2635’ set a minimum 100’ cement plug from 2635’ to 2528’. TOH, WOC.

8. Run bond log to find free point and cut 6 5/8” casing prior to setting plug across 6 5/8” casing seat. If free point was found to be shallower, and the entire casing string will not pull. Perforate all additional plugging locations as determined by bond log.

9. ***Note*** Casing free point will determine whether the casing is perforated and cemented at all plugging locations, an additional plug may be required across casing cut achieved.

10. Tag top of plug. Add cement if needed.

11. TIH with tubing, spot 6% gel from 2528’ to 1962’.

12. Set cement plug from 1962’ to 1753’ to cover where well bore was shot and cover casing seat. TOH, WOC.

13. Tag top of plug. Add cement if needed.

14. TIH to estimated 1753’set 6% gel spacer from 1753’to 1315’. Pull tubing to 1315’ and set a minimum of 100’ cement plug from 1315’ to 1215’ covering saltwater zone TOH, WOC.

15. Tag top of plug add cement as needed.

16. TIH with tubing, spot gel from 1215’ to 1026’ set a minimum 100’ cement plug from 1026’ to 926’.

NOTE.. Any casing found in the well without cement in the annulus that cannot be retrieved must be perforated and cemented at all saltwater, freshwater, coal, oil, gas, casing shoe and elevation points.

Site Reclamation Plan

All plugging equipment will be removed from the site. Culverts will be removed and disposed of.

All slopes will be reestablished as near original grade as possible. The well site and road will be limed, fertilized, seeded and mulched in accordance with the terms of the permit and Reclamation

31.0 Power Swivel

The power swivel is a mechanical device that suspends the weight of the drill string. It is designed to allow rotation of the drill string beneath it, conveying high volumes of high-pressure drilling mud between the rig's circulation system and the drill string.

The power swivel must be a minimum of a twenty-five (25) ton hydraulic unit. The Power

Swivel must include the Power Unit with hoses and connections, also includes two Saver

Subs (one for Drill Pipe and one for Drill Collars).

This item is bid per hour of use.

32.0 Concave Mill

A mill is a tool that grinds metal downhole, typically to remove junk or parts of a casing string.

Mills will include subs needed.

1. 4 ¾ inch mill

Item is bid per mill.

33.0 Mud/mixing pump

A mud pump is a reciprocating piston/plunger pump designed to circulate drilling fluid under high pressure down the drill string and back up the annulus.

This pump must have a minimum of five (5) inch diameter with a seven (7) inch stroke length or larger, and able to supply volumes and pressure as needed. It will include hoses and connections capable of handling the pump output.

Item is bid per hour of use.

34.0 Logging, Bond with CCL

Description

Logging includes all labor, equipment, and material necessary to determine various characteristics of the well and using probative tools suspended from a wire line. Bond logs with casing collar locator (CCL) allows for a determination of the cement bond quality behind the casing and identify the free point of any casing strings present in the well.

Logging includes all labor, equipment, and materials required to complete logging run.

Item is bid per log.

34.5 Wireline Shot/Perforation, Collar Buster

The perforation, severance, or separation of pipe by wireline tools equipped with explosive shots (i.e., collar busters, perf balls) or other wireline mechanical means (i.e., jet-cutter), typically in conjunction with a bond log.

Functional Requirements:

The following sizes may be needed:

1. Up to 8 ⅝”

This item is bid per run.

35.0 Tie Mats

Tie mats are portable structures used to support the weight of heavy equipment.

Minimum size 4’ x 12’. Tie mats may be constructed of natural or synthetic materials.

Item is bid per mat for the duration of the project.

36.0 Topsoil

The excavated area may need to be backfilled with topsoil. The quantity of topsoil will be based on the quantity of soil disposed minus the amount of gravel that is placed along the well.

Item is bid per ton and includes excavation from the borrow area, if applicable, transportation to the site and equipment and labor to level the site. At no time may an individual worker’s hours be billed simultaneously for work completed under this item and any other labor-based item allowing for hourly rate payments (e.g., ‘Rig and Crew’ or ‘Heavy Equipment.’)

37.0 Culvert

A culvert is a pipe intended to channel water.

It is anticipated that culverts of the following size will be needed (diameter x length):

1. 36” x 20’ (thirty-six inches by twenty feet)

2. 24” x 20’ (twenty-four inches by twenty feet)

3. 16” x 20’ (sixteen inches by twenty feet)

Culvert may be metal or HDPE.

Item is bid for each culvert.
