DISTRICT-WIDE ASBESTOS CONSULTANT SERVICES

Section Question

1 Page 13 of RFP

Please clarify the Vendor’s Business History on Page 13 of the RFP. Does this include all work associated with the responding company/firm or just the office location of the firm within District 7? Any work associated with the responding company that relates to the services we are requesting. Also, can you define what is considered governmental work? Governmental work is work done for local, state and federal agencies.

Page 23 of RFP, item 4

Can you provide a link or attach the State of Florida Department of Transportation Initial Building Survey Review (Form # 575-060-035)? Form is attached to the email.

3 Page A-5, Item 11

Please confirm or clarify that 2 separate asbestos survey reports will be required as indicated in Item 11 on Page A-5 of the Scope of Work? An asbestos survey is required and if necessary, a work plan. IF the asbestos survey comes back positive then an O&M plan will be necessary.

Page 25 of RFP, item B - Operation and Maintenance plans

What is O&M Emergency Site Security? Is that performed in conjunction with a State of Florida licenses asbestos abatement contractor? This is either armed or unarmed security. This is in conjunction with our asbestos abatement Vendor.

Page 27 of the Scope of Work, items E - final clearance reports

Is the Vendor responsible for the post-job submittals including manifests of the asbestos abatement and/or demolition contractor? The asbestos consultant does sign off in this case. What if the contractors do not provide the documents within 10 days as required on Page 27 of the Scope of Work? That will be at the discretion of the Department’s Contract Manager. See Section 3, A and 6, A – E of the Standard Written Agreement.

6 Pricing document

What is the difference between a Brief O&M Report and Full O&M Report as requested in the Price Proposal? The brief O&M Plan shall be developed by a licensed asbestos consultant and shall simply and briefly address the location and type of ACM present and summarize any special material handling requirements.

A full O&M report is a longer more detailed report.

7 Pricing document

Why is there a quantity of “2” for the development of the Master Specifications?

See Exhibit “C”, Price Proposal #3, The estimated quantities are for determining low proposal and have no bearing on the actual quantities that may be utilized under this contract.
