TECHNICAL QUESTIONNAIRE

PLEASE NOTE: IN ADDITION TO PRICING PROVIDED ON THE BID/PRICE SCHEDULE, YOUR
RESPONSES ON THIS QUESTIONNAIRE (AND YOUR RESPONSES ON THE PAST PERFORMANCE
QUESTIONNAIRE) ARE CRITICAL TO THE SELECTION OFFICIAL WHEN EVALUATING A
PROPOSAL FOR AWARD. INADEQUATE OR VAGUE RESPONSES MAY RESULT IN A LOWER
RATING (PLEASE SEE SECTION M OF THE SOLICITATION FOR EVALUATION INFORMATION).
YOU MAY USE ADDITIONAL PAGES FOR YOUR RESPONSES, IF NEEDED.
SUBFACTOR #1 – Technical Approach
Technical approach is your means and methods to complete the technical aspect of required work in painting and carpet installation as described in the project specifications. Submit a written narrative limited to 5 pages describing your technical approach to the specific aspects below. Additional pages will not be evaluated. Pictures can be included and will not be counted towards the page limitation.
Approach to Critical Contract Requirements: Describe your approach to successfully accomplish interior painting and carpet installation in accordance with requirements outlined in the Project Specifications and Drawings within proposed contract performance period and budget.
Please outline your plan for transporting materials and equipment. Also, please explain how you will staff the project, given seasonal ferry restrictions and housing considerations.
Please include methods for protecting park exhibits and interpretive displays in exhibit area, and windows in visitor center.
SUBFACTOR #2 – Management Approach
Management Approach is the organizational structure, equipment/assets and business processes to manage, control, and oversee your firm's operations. Submit a written narrative limited to 3 pages describing your management approach to the specific aspects below. Additional pages will not be evaluated. Pictures can be included and will not be counted towards the page limitation. There is no preprinted form for this submittal.
Organization Chart: Submit an organizational chart identifying the project team with major subcontractors and key personnel, identifying specific roles, responsibilities, and lines of authority.
Owner & Regulatory Entity Coordination: Describe proposed coordination with the project owner (National Park Service), the State of Ohio, the Village of Put-in-Bay, and regulatory entities to complete work within all parameters of the solicitation if this is applicable for this project.
SUBFACTOR #3 – Key Personnel
Submit resumes for key personnel as identified below with experience in interior painting and carpet installation. All personnel proposed are subject to substitution requirements contained in this solicitation.
Project Manager Superintendent Construction Management (field staff) On-Site Quality Control Specialist On-Site Safety Specialist/Manager
SUBFACTOR #4 – Project Experience
Submit "Project Experience" addressing specific experience working in interior painting and carpet installation in the below section/box, preferably a minimum of 3 and/or a maximum of 5, relevant projects completed in the last 7 years. Specifically, include any work using scaffolding for painting or other projects.
NOTE: Submit the same projects for Past Performance as submitted under this factor.
SUBFACTOR #5 – Schedule
Submit your proposed construction schedule for THIS SPECIFIC PROJECT showing completion within the required period of performance contained in this solicitation with accompanying schedule narrative.
Submit your schedule so that it is clear and concise, addressing the following: major milestones, explain schedule logic, critical path activities, procurement lead-times and key considerations and constraints.
There is no preprinted form for this submittal.
SUBFACTOR #6 – Safety
Submit an overview of your company’s Construction Safety Program and an assessment of the safety risks associated with this construction project. Submit a written narrative limited to 10 pages describing your safety plan for a project of this nature.
Include the names, resumes, and specific safety training completed (i.e., OSHA, etc.) for the proposed Project On-Site Safety Specialist/Manager for this project.
Include safety measures for painting using scaffolding for projects up to 3 stories in height.
