About this file
This is a Sales and Use Tax Construction Contract Exemption Certificate form issued by the State of Ohio, specifically designed for the City of Toledo's construction projects. The form is used to document and certify that tangible personal property and materials purchased for construction projects are exempt from sales and use tax when incorporated into real property. The certificate applies to the Telegraph Road & Alexis Road Resurfacing & Multi-Use Path project (LUC-US24-28.3, PID 117877) and must be signed by the owner/contractee, the City of Toledo political subdivision representative, the prime contractor, and any subcontractors involved in the project. The form requires identification of the contract, exact job location, and project name as it appears on contract documentation, with the original certificate retained by the prime contractor and copies maintained by the owner, contractee, and all subcontractors.
The certificate identifies multiple categories of real property and construction activities eligible for tax exemption under Ohio Revised Code, including charitable buildings, agricultural structures, sports facilities, hospital facilities, computer data centers, megaprojects, and properties owned or to be owned by government entities or qualifying organizations. The form is effective as of January 1, 2026, and has been signed by the City of Toledo's Commissioner of Purchases and Supplies at One Government Center, Suite 2000, Toledo, Ohio 43604. No pricing terms, set-asides, or funding information is contained within this exemption certificate form; it serves solely as a tax compliance document required for bid submission on this City of Toledo construction project.
