About this file

This is a Sales and Use Tax Construction Contract Exemption Certificate issued by the State of Ohio for the City of Toledo's Collins Park Bath House and Pool Demolition project. The certificate is a pre-filled form designed to document the exemption of tangible personal property from sales and use taxes when such property is incorporated into eligible construction projects. The document identifies the contractee as the City of Toledo and requires signature and acknowledgment from the owner/contractee, a government official, the prime contractor, and any subcontractors involved in the project. The certificate must be retained by the prime contractor in original form, with copies maintained by the owner, contractee, and all subcontractors for use when purchasing tax-exempt materials from suppliers.

The exemption certificate applies to construction or improvement projects meeting specific criteria under Ohio Revised Code, including real property owned or to be owned by the United States government, the State of Ohio, or an Ohio political subdivision such as the City of Toledo. The form is dated January 1, 2026, and is administered through the City of Toledo's Commissioner of Purchases and Supplies at One Government Center, Suite 2000, Toledo, Ohio 43604. Bidders for this project must submit the Tax Exemption Certificate along with other required documentation including the Construction Project Paperwork Packet, proof of insurance, worker's compensation certificates, and a bid guaranty through the City's PlanetBids portal to be considered for award.
