About this file

This is a Paperwork Schedule document issued by the City of Toledo, Ohio, outlining a streamlined procurement approach for bidders beginning with bids advertised on or after March 13, 2026. The document establishes three categories of required documentation: City Paperwork (submitted once for the duration of all contracts), Annual Paperwork (submitted once per calendar year), and Project Paperwork (submitted with each individual bid). City Paperwork includes the Vendor Registration Form, Business Tax Registration Form, notarized Child Support Affidavit, and Vendor Utility documentation for companies new to doing business with the City. Annual Paperwork encompasses the W-9, Certificate of Insurance naming the City of Toledo as an additional insurer, Workers' Compensation Certificate, EEO Compliance certification, Debarment Certification, and Living Wage Affidavit updated no later than February 1st. Project Paperwork required with each bid submission includes the Primary/Subcontractor Agreement Certificate, Acknowledgement & Acceptance of Terms & Conditions, Living Wage Contract Addendum, and Best Bid Criteria when applicable.

The schedule significantly reduces documentation burden for repeat bidders by requiring one-time submission of foundational paperwork and annual certifications, with only project-specific documents needed for subsequent bids within the same year. The example provided clarifies that a company submitting its first bid on March 13, 2026, must submit all three paperwork categories; however, subsequent bids in 2026 require only Project Paperwork submission. Companies are responsible for notifying the City of any changes to submitted information and updating applicable documentation accordingly. Questions regarding paperwork submission should be directed to purspplies@toledo.oh.gov, and all submissions are processed through the City's PlanetBids portal.
