Supplier Diversity Program (SDP) Plan Form
This is a mandatory Supplier Diversity Program (SDP) Plan Form required by the Massachusetts Executive Office of Health and Human Services (EOHHS) for the RFR for Personal Care Attendant Identification Card Services. All bidders must complete and submit this form with their bid response to be eligible for contract award. The form requires bidders to commit to spending a minimum of 1% of contract sales with certified diverse businesses, unless the RFR specifies otherwise. Bidders may categorize their SDP spending commitment as either subcontracting (goods or services directly related to the contract) or ancillary products and services (goods or services supporting general business operations), or divide their commitment between both categories. The form mandates completion of six sections: bidder contact information, SDP spending commitment percentage, proposed SDP partners with their certifications and relationship types, business practices supporting diverse suppliers, additional supplier diversity initiatives, and authorized certification by company officials under penalties of perjury.
Proposed SDP partners must be certified by eligible organizations such as the Supplier Diversity Office (SDO), Small Business Administration (SBA), state or city certification offices, the National Minority Supplier Development Council (NMSDC), or the Women's Business Enterprise National Council (WBENC). Eligible certifications include Minority Business Enterprise (MBE), Woman Business Enterprise (WBE), Veteran-Owned Business Enterprise (VBE), Service-Disabled Veteran-Owned Business Enterprise (SDVOBE), Disability-Owned Business Enterprise (DOBE), Lesbian, Gay, Bisexual, or Transgender Business Enterprise (LGBTBE), and associated nonprofit organization certifications. Self-certification is not acceptable. Bidders certified as diverse businesses by the SDO or another organization remain required to submit an SDP commitment. Bidders may earn additional evaluation points for providing optional information regarding prior-year spending with proposed SDP partners and existing business practices supporting supplier diversity. The form must be submitted as an Excel file attachment to the bid response and was last updated in October 2026.
