About this file
Summary
This is a Model Contract and Request for Responses (RFR) issued by the Massachusetts Executive Office of Health and Human Services (EOHHS) for Personal Care Attendant (PCA) Identification Card Services. The contract encompasses processing, printing, mailing, and customer service support for PCA identification cards distributed to eligible personal care attendants enrolled in the MassHealth PCA program. The contractor will be responsible for receiving requests from consumers or surrogates, validating PCA eligibility through the contracted Fiscal Intermediary, producing durable wallet-sized cards meeting EOHHS specifications, and providing multilingual customer assistance via telephone and email. Implementation deliverables must be completed within ten to fifteen business days prior to the contract start date, including system readiness, card proof approval, production capability demonstration, and HIPAA-compliant disaster recovery plan approval. The contract term begins on the effective date of execution and extends through June 30, 2028, with options for EOHHS to extend for up to three additional years or further reasonable increments as mutually agreed by both parties.
Compensation consists of a one-time start-up fee upon completion and acceptance of implementation deliverables, followed by monthly fees structured on a volume-based tiered pricing model based on the number of cards produced and mailed monthly. The monthly fee covers all services including request processing, eligibility validation, production materials, printing, packaging, postage, customer service, reporting, and all necessary systems, staffing, facilities, and equipment. The contractor must meet four Service Level Agreements: process and produce at least 95% of cards within five business days of receiving complete requests; maintain 95% card accuracy against EOHHS specifications with defective cards reproduced at no additional cost; mail at least 95% of completed cards within five business days; and respond to at least 90% of customer inquiries within two business days. Failure to meet SLA targets for two consecutive months triggers corrective action plans, and EOHHS reserves the right to impose financial penalties, liquidated damages, or pursue contract suspension or termination for poor performance.
