About this file

This is a Supplemental Bidder Attestation form issued by the State of Rhode Island Division of Purchases for use in response to the MPA-534 Elevator Service and Repairs Master Price Agreement solicitation. The form requires authorized signatories from bidding vendors to certify that their company and all parent corporations, subsidiaries, affiliates, and subcontractors have no conflicts of interest as defined under Rhode Island General Laws Chapter 14 of Title 36 with any state official, officer, or agency involved in the RFP process. Vendors must also attest that they did not materially participate in or receive consultation regarding the technical requirements, specifications, or other aspects of the solicitation's development. The attestation explicitly states that prior responses to Requests for Information do not constitute participation or consultation that would disqualify a vendor from future RFP participation. Proposals for the elevator service, repair, and maintenance contract are due August 1, 2028 at 1:00 PM EDT through the Ocean State Procures electronic submission system, with the initial contract period running approximately three years from November 2026 and potential renewal for up to two additional 12-month periods.

The attestation form must be completed and signed by an authorized representative who certifies under penalty of perjury that the information provided is true and accurate. Simply being a current or former state vendor does not automatically create a conflict of interest requiring vendor disqualification. The form requires the vendor's legal entity name, the solicitation number and title, the authorized signatory's printed name and signature, and the date of attestation. Vendors previously awarded contracts under the prior elevator solicitation RFPM26006450 are exempt from resubmitting proposals for this master price agreement.

1 A “conflict of interest” occurs when private interests or relationships interfere in any way with the interests of the

State. Simply being a current vendor or a past vendor of the State does not in itself cause a conflict requiring preclusion. Additionally, formal responses to a Request for Information (RFI) issued by the Division of Purchases does not constitute “participation” or “consultation” with respect to a future RFP and does not disqualify a vendor from future participation from a subsequent RFP.
