About this file
This is a Supplemental Bidder Attestation form issued by the Rhode Island Division of Purchases for use in conjunction with the Department of Corrections' Request for Proposal (RFP) for complete chemical cleaning, manufacturing, packaging, sales, distribution, and training services. The form, revised November 29, 2024, requires authorized signatories to certify that their company and all affiliated entities, subsidiaries, and subcontractors do not have conflicts of interest with any state official, officer, or agency involved in the RFP process, and have not materially participated in or been consulted regarding the RFP's requirements, technical aspects, or formation.
The attestation mandates that bidders confirm the RFP does not relate to any audit, examination, independent verification, review, or evaluation of their organization's work, financials, or operations previously performed for the State of Rhode Island. The form clarifies that being a current or past state vendor does not inherently constitute a disqualifying conflict of interest, and that responses to prior Requests for Information do not constitute participation that would disqualify vendors from future RFP participation. Bidders must complete all blank fields including their entity name, authorized signatory name and signature, solicitation number and title, and the attestation date before submission.

1 A “conflict of interest” occurs when private interests or relationships interfere in any way with the interests of the
State. Simply being a current vendor or a past vendor of the State does not in itself cause a conflict requiring preclusion. Additionally, formal responses to a Request for Information (RFI) issued by the Division of Purchases does not constitute “participation” or “consultation” with respect to a future RFP and does not disqualify a vendor from future participation from a subsequent RFP.
