About this file

This is a Subcontractor Summary Worksheet instruction document from the City of St. Petersburg, Florida, applicable to the Oversized Vehicle Towing contract opportunity. The worksheet serves as a mandatory submission requirement for all bids and documents the percentage of work that may be subcontracted, along with detailed information about each intended subcontractor. Prime contractors must complete the form by providing their company name, the name and date of the employee completing the form, and comprehensive details for each subcontractor including the subcontractor's legal name, primary business address, project supervision contact information, trade classifications, subcontract amount, and tier level (first or second tier only). The worksheet allows bidders to indicate that no subcontractors will be used if applicable. Subcontractors may only be added or deleted after project award under specific circumstances, including subcontractor failure to execute a contract, lack of appropriate licensing, bankruptcy or insolvency, ineligibility, failure to perform, unsatisfactory work performance, or substantial delays or disruptions to project progress.

The document imposes restrictions on subcontracting percentages and multi-tier subcontractors, though these restrictions do not apply to FDOT LAP or federally funded projects. Small Business Enterprise (SBE) or Minority/Women Business Enterprise (MWBE) subcontractors used to meet mandatory SBE percentage requirements must be replaced with another SBE subcontractor if substituted, unless the prime contractor demonstrates compliance through alternative means. Any requests for subcontractor changes must be submitted in writing to the Procurement & Supply Management department, specifically to the Contracts Compliance Manager at Stephanie.Swinson@stpete.org. The worksheet includes columns for tracking additions and deletions with corresponding reason codes and city approver sign-off and dates.
