About this file
This is a Subcontractor Summary Worksheet instruction document from the City of St. Petersburg, Florida, establishing guidelines for subcontractor participation in municipal projects. The worksheet serves as a mandatory submission component with all bids and requires prime contractors to disclose all intended subcontractors, including their names, addresses, contact information, and trade classifications. The document specifies that 80% of the Shelter contract work may be subcontracted, with only first and second-tier subcontractors permitted unless otherwise noted in contract documents. Prime contractors must provide the name and date of the employee completing the form and identify whether subcontractors are Small Business Enterprises (SBEs) or Minority/Women Business Enterprises (MWBEs).
The worksheet establishes strict restrictions on subcontractor changes post-award, permitting substitutions only under specific circumstances including contractor failure to execute agreements, lack of proper licensing, bankruptcy, insolvency, ineligibility, performance failure, unsatisfactory work quality, or substantial project delays. SBE subcontractors used to meet percentage requirements must be replaced with other SBEs unless the prime contractor demonstrates alternative compliance methods. Any requested changes to the subcontractor list must be submitted in writing to the City's Procurement & Supply Management office, specifically to the Contracts Compliance Manager at Stephanie.Swinson@stpete.org. The instructions note that subcontracting restrictions and multiple-tier limitations do not apply to FDOT LAP or federally funded projects.
