About this file

This is a Subcontractor Summary Worksheet instruction document from the City of St. Petersburg, Florida, designed for use with construction and service contracts. The worksheet requires prime contractors to disclose all intended subcontractors, including their names, business addresses, contact information, trade classifications, and subcontract amounts. Bidders must complete and submit this form with their bids, and if no subcontractors will be used, they must provide a written statement to that effect. The document establishes that subcontractors may only be changed or substituted after contract award for specific reasons, including failure to execute a contract, lack of appropriate licensing, insolvency, ineligibility, failure to perform, substantially unsatisfactory work, or substantial delays to project progress. All requests for subcontractor changes must be submitted in writing to the Procurement & Supply Management department, attention Contracts Compliance Manager at Stephanie.Swinson@stpete.org.

The worksheet permits both first and second-tier subcontractors unless otherwise specified in contract documents, and restrictions on subcontracting percentage do not apply to FDOT LAP or federally funded projects. Small Business Enterprise (SBE) subcontractors used to meet SBE percentage requirements must be replaced with other SBE subcontractors if substitution is necessary, unless the prime contractor demonstrates that SBE requirements are being met through alternative means. The document includes designated fields for tracking whether subcontractors are SBE or MWBE certified, the specific reasons for any additions or deletions, and city approval signatures with dates, maintaining a complete audit trail of all subcontractor changes throughout the contract performance period.
