About this file

This is a Subcontractor Summary Worksheet instruction document issued by the City of St. Petersburg, Florida, for the Gladden Park Surveillance System Wiring & Installation Project. The worksheet is a mandatory submission requirement with all bids and establishes the framework for subcontractor utilization on this project. Bidders must enter their company information, identify all intended subcontractors with complete legal names and business addresses, and provide contact information for the employee responsible for project supervision. The document specifies that up to 80 percent of contract work may be subcontracted, with only first and second tier subcontractors permitted unless otherwise noted in contract documents. Subcontractors must be classified by trade and listed with their subcontract amounts on the worksheet.

Once a prime contractor is awarded the project and the pre-construction meeting is held, subcontractors may not be changed or substituted except under specific circumstances including failure to execute a contract at the specified price, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform work, failure to perform, substantially unsatisfactory work performance, or substantial delays to project progress. Requests for subcontractor changes must be submitted in writing to the City's Procurement & Supply Management office, specifically to the Contracts Compliance Manager at Stephanie.Swinson@stpete.org. Small Business Enterprise (SBE) or Minority and Women-Owned Business Enterprise (MWBE) subcontractors used to meet SBE percentage requirements must be replaced with other SBE subcontractors if substitution becomes necessary, unless the prime contractor demonstrates that SBE requirements are being met through alternative means. The worksheet was last revised in January 2020.
