Subcontractors must forward notification of sub-subcontractor affidavits to the prime contractor within five business days of receipt, including any cascading notices from lower-tier subcontractors.
The affidavit requires the subcontractor to provide its federal work authorization user identification number and date of authorization, along with the name of the subcontractor, project name, and the public employer (Douglas County Board of Commissioners). The form must be executed under penalty of perjury by an authorized officer or agent and notarized. This document serves as a compliance verification mechanism to ensure that all entities performing work on the Douglas County contract maintain proper work authorization documentation and participate in the mandatory federal E-Verify system.
