About this file
This is a Subcontractor Affidavit form required by the Douglas County Board of Commissioners in Georgia for the I-20 Landscape Maintenance Bid project. The affidavit verifies that subcontractors comply with O.C.G.A. § 13-10-91, which mandates the use of the federal E-Verify work authorization program. Subcontractors must confirm their registration with E-Verify, provide their federal work authorization user identification number and date of authorization, and commit to maintaining E-Verify compliance throughout the contract period. The form requires subcontractors to ensure that any sub-subcontractors also provide compliant affidavits and establish a notification protocol whereby the subcontractor must forward notice of receipt of sub-subcontractor affidavits to the prime contractor within five business days.
The affidavit must be executed before a notary public and signed by an authorized officer or agent of the subcontracting firm. The document serves as a legal declaration under penalty of perjury, confirming the subcontractor's adherence to Georgia's work authorization requirements. This form is a prerequisite for participation in the landscape maintenance project at four I-20 interchanges in Douglas County and reflects the state's commitment to employment eligibility verification across all contractual tiers.
