About this file

This is a Subcontractor Affidavit form required by Douglas County Board of Commissioners under O.C.G.A. § 13-10-91(b)(3). The affidavit verifies that subcontractors engaged in physical performance of services on behalf of the county have registered with and are authorized to use the E-Verify federal work authorization program. Subcontractors must provide their federal work authorization user identification number and date of authorization as part of the affidavit execution. The form requires completion of the subcontractor name, project name, execution date, location, and authorized officer signature, followed by notarization. Subcontractors must continue using E-Verify throughout the contract period and may only contract with sub-subcontractors who also submit required affidavits.

Subcontractors are obligated to forward notice of affidavits received from sub-subcontractors to the primary contractor within five business days of receipt and must notify the contractor of any notices received from sub-subcontractors who have further contracted with additional sub-subcontractors within the same five-business-day timeframe. The affidavit is executed under penalty of perjury and must be notarized by a notary public. This requirement ensures compliance with Georgia state law regarding federal work authorization verification for all contracted service providers working on Douglas County projects.
