Contractor Affidavit Summary
This document is a Contractor Affidavit required under O.C.G.A. § 13-10-91(b)(1) for contracts with Douglas County, Georgia. The affidavit serves as a compliance verification document whereby contractors attest to their adherence to Georgia's federal work authorization requirements. The affidavit does not specify particular products, services, project timelines, quantities, response dates, site visits, bidder meetings, award dates, or contract terms. Instead, it functions as a mandatory compliance instrument that contractors must execute and have notarized prior to contract award or execution.
The affidavit requires contractors to register with and utilize E-Verify or any subsequent federal work authorization replacement program throughout the contract period, including any renewals or extensions. Contractors must maintain continuous E-Verify compliance and notify Douglas County immediately if they cease participation, with non-compliance constituting material breach of contract. Contractors are additionally required to ensure all subcontractors provide compliant affidavits and must submit copies of affidavits, drivers' licenses, and identification cards to Douglas County within five business days of receipt. The document contains no pricing terms, set-asides, incumbent contractor information, or funding source details, as it is solely a compliance certification requirement.
