About this file
This is a Request for Quotation (RFQ) issued by the Ohio Department of Natural Resources (DNR) for repairs to floor drains at Stonelick State Park in Pleasant Plain, OH. The scope involves replacing floor drains and traps in 10 bathrooms throughout the park that were damaged when epoxy from a 2024 floor refinishing project clogged the drains and traps. The work requires cutting and demolishing 2'x2' concrete sections, removing existing drains/traps, verifying pipe conditions, installing new components, and restoring concrete flooring with proper drainage slopes. The completion date is set for March 1, 2025, with work to be performed during normal business hours (M-F 8am-5pm). Prospective bidders are encouraged to schedule site visits with the NRT Supervisor/Maintenance Manager prior to submitting bids.
The contract will be awarded to the lowest responsive and responsible bidder based on total lot price. The lowest apparent bidder must provide examples of two similar completed projects with photos and references, as well as complete a pre-award meeting with DNR staff. All invoices must be submitted to Ohio Shared Services with specific documentation requirements including PO number, supplier details, service dates, itemized charges, and facility information. The contract period runs through June 30, 2025, with any services after that date requiring a new PO. The images included show floor drains with epoxy damage radiating outward from the drain covers, demonstrating the scope of repair needed.
