About this file

Short-Term and Long-Term Disability Benefits - Colorado

This document outlines the Short-Term and Long-Term Disability Benefits program for the State of Colorado. The file establishes the framework for disability insurance coverage, detailing the types of benefits available to eligible employees and the terms under which claims are processed and paid. The program encompasses both short-term disability (STD) benefits for temporary inability to work and long-term disability (LTD) benefits for extended periods of incapacity. The scope includes definitions of qualifying disabilities, benefit eligibility criteria, waiting periods, maximum benefit durations, and the claims administration process. The document specifies the coordination of benefits with other insurance programs and outlines the employer's obligations regarding employee notifications and compliance with applicable regulations.

The benefits structure provides specific replacement income percentages and maximum weekly or monthly benefit amounts for both STD and LTD coverage. The program includes provisions for benefit calculations based on employee earnings and service tenure, with defined exclusions and limitations. Cost-sharing arrangements between the employer and employees are established, along with procedures for claim submission, verification, and appeal processes. The document addresses integration with workers' compensation, social security benefits, and other disability-related programs to prevent duplication of payments. Administrative responsibilities are clearly delineated, including record-keeping requirements, notification procedures, and compliance documentation necessary for program operation and employee access to disability benefits.
