About this file

Summary

This is an Offeror Questionnaire document for a Colorado state contract opportunity related to Short-Term and Long-Term Disability Benefits. The questionnaire is designed to collect standardized information from potential offerors bidding on this contract. The document appears to be formatted as an Excel spreadsheet that requires offerors to provide responses to questions about their qualifications, experience, and capability to deliver disability benefits services to the state.

The questionnaire likely contains sections requesting information about the offeror's organizational structure, relevant experience with disability benefits administration, financial stability, staffing qualifications, service delivery capabilities, and compliance with state requirements. This tool serves as a preliminary evaluation mechanism to assess whether potential vendors meet baseline qualification criteria before full proposal evaluation. Offerors must complete and submit this questionnaire as part of their response to the procurement opportunity, with responses used to determine eligibility and competitiveness for the Short-Term and Long-Term Disability Benefits contract.
