Areas of the building that are not accessible at the time of the survey or otherwise not inspected are to be documented in the final report and all maps and drawings.

b. Bulk samples taken from all roof areas of any building or structure of the CMCVAMC must be overseen by a roofing contractor approved by the CMCVAMC to ensure all damage to facility roofing systems caused by sampling are repaired immediately. The contractor is to include these costs into their proposal.

c. The contractor will conduct a thorough evaluation of all crawlspaces, tunnels, interstitial floors and similar areas throughout the facility for the presence asbestos debris. In some areas this will require taking samples of soil to confirm the presence of asbestos containing debris in the soil.

d. The contractor shall collect all samples of suspected ACM in accordance with VHA Directive 7714, EPA, AHERA, NESHAP, State of Pennsylvania, the City of Philadelphia, and OSHA regulations and in a manner preventing airborne fiber exposure to the surveyor and facility personnel. Samples taken in occupied office spaces will be taken in locations that minimize the risk of potential exposure to occupants and visual impact of the resulting sample location. The contractor shall restore visible sample locations to a condition as close to original as possible. The contractor shall encapsulate all locations to prevent any subsequent release of asbestos fibers after sample collection. The contractor shall mark sample locations, an exception to this are conspicuous sample locations within occupied areas. Records and descriptions of non-labeled occupied space samples will be provided.

l. Sample costs shall be quoted on a per sample basis. The number of samples required shall be based on the number of previously suspect materials not sampled and the number of negative or positive materials that require re-sampling. The Medical Center

TPOC shall be consulted on the number of samples to be submitted for analysis.

m. PLM results ranging from less than one percent to ten percent may require additional analysis such as transmission electron microscopy (TEM). Therefore, the Contractor will make arrangements with the laboratory to retain such samples in the event that additional analysis is requested. Samples shall be retained by the laboratory for at least ninety days in the event that reanalysis of any samples is required as determined by the contractor or CMCVAMC staff.

n. The contractor may implement a stop at first positive strategy of analysis to conserve limited funding resources.

o. All asbestos bulk samples that are determined to be less then 1% asbestos through PLM analysis will be reanalyzed utilizing either point counting methodologies or TEM analysis by an accredited laboratory to confirm asbestos content.

p. Whenever possible asbestos bulk samples will not be composited into one sample. For example, all joint compound and sheetrock shall be sampled and analyzed individually.

q. Throughout the period of performance of this contract the contractor may be requested to review, inspect, and sample any suspect asbestos containing materials that are discovered after the initial inspection that are not otherwise identified on the final report as being determined as asbestos free. The contractor will be responsible for confirming if this material is asbestos containing and updating the final report and management plan accordingly.

r. The contractor will be responsible for documenting all asbestos abatements and updating all reports and documentation accordingly including the final report and asbestos management plan.

X. Inspection/Sampling/Evaluation Procedure

Narrative description and photographic evidence of each asbestos HA is required.

f. Sample costs shall be quoted on a per sample basis. The number of samples required will be based on the number of homogeneous areas, contractor experience and the representative contractor will consult with contractual representative on number and type of samples taken. Some materials may be presumed to contain asbestos in accordance with AHERA/ASHARA.

g. Identify the appropriate management action to protect the health and safety of the building occupants.

h. For each suspect material, collect bulk samples for analysis in accordance with VHA Directive 7714, 40 CFR 763.83, OSHA 29 CFR 1910.1001 and all other applicable regulations.

XI. Reinspection

a. Any follow-up inspections or surveys of suspect asbestos containing materials required because they were not identified/missed by the contractor in the initial full building survey report will be conducted at the expense of the contractor. If necessary, the TPOC will provide in writing areas to not be sampled/surveyed in the initial full building survey report.

These areas may be sampled at a later date as identified by the TPOC and billed at the contracted rate.
