About this file
This is a Statement of Work (SOW) for the decommissioning and remediation of a Weaver EC105B/106B Twin Post Heavy Duty Lift at the USDA Dairy Forage Research Center in Prairie du Sac, Wisconsin. The project involves complete removal of the non-functioning lift, soil contamination testing, backfill and compaction, and installation of new heavy-duty rated concrete flooring that ties into and matches the existing shop floor.
Key technical requirements include conducting a site assessment; submitting a detailed demolition plan with safety protocols, equipment specifications, and disposal methods prior to work commencement; acquiring Wisconsin Department of Safety & Professional Services (DSPS) permits and approvals; performing removal by credentialed personnel per SPS 305.991; properly disposing of used oil in accordance with federal, state, and local regulations; executing straight saw cuts with right angles to maintain parallel and square lines; removing all lift components and sub-assemblies for disposal or recycling; conducting soil testing before backfill begins; backfilling to sub-slab level with appropriate compaction; tying new concrete with rebar/mesh to existing flooring; and matching the finish texture of new concrete to the existing floor. Work must be performed during normal business hours (7:00 am–5:00 pm, Monday–Friday, excluding federal holidays). The contractor's site superintendent must demonstrate prior experience removing underground lifts with proof of past performance. Key deliverables include the demolition plan and permit copies, soil test results, backfill material and concrete strength specifications, and a final recycling content report documenting pounds of material recycled, all to be submitted via email to the Contracting Officer's Representative (COR) at specified intervals before and after work phases.

General Information
1.0 Background and Purpose
The Dairy Forage Research Center, located at S8822 Sunset Drive, Prairie du Sac, WI, has an existing Weaver EC- 105B/106B Twin Post Heavy Duty Lift that is no longer functioning and needs to be decommissioned. The lift is in a maintenance building for tractors and farm vehicles. The goal is to have the contactor remove the lift, test the soil for contamination, restore/prep the area with fill and compaction, tie in/reinforce the new concrete to the slab with same thickness and texture of the existing shop floor. Saw cuts/demo should be done with the goal to keep lines parallel and square as possible. The lift is comprised of the main portion which is L-shaped of about 4’X18’X18”wide ~ 7 ½ deep. A smaller ~18”x3’ section. See attached Parts Manual for exploded diagram. New concrete shall be rated for heavy duty shop floor traffic. Wisconsin DSPS approval and permitting will need to be completed by contractor.
